Post chronic pelvic, hip neuromuscular pain from traumatic childbirth

Seeking input/comparing notes regarding 10 years of excruciating pain resulting from 4th degree tear and damage to sacrum during traumatic childbirth. Raw "toothache"-like pain on left side of pelvic floor with attendant muscular pain in left hip and left side of sacrum. Pain radiating down left leg into foot and up left arm into shoulder. 3 years of physical therapy have led to corrected alignment of pelvis at sacrum but pain levels remain. Currently doing myofascial work and perineal ultrasound; pain becoming much more specific but level not subsiding. Any clues out there about how to end this nightmare?
