I had very similar problems, and I take heavy duty relaxers for periodic limb movement. My sleep doc asked me to ignore the clock. She said unplug it if you can, if you need the alarm (which I do for work) then turn it around so you can't see it from bed. She said it would be a little odd at first, and you immediately dismiss it as stupid, but it will work. She said that we get into cycles of waking and sleeping, and taking the clock out of the situation will help to break the cycle.
Sure enough, within a week I was waking up less during the night, and even then only to use the restroom or get some water. I now sleep with my clock turned towards the wall every night.
