Sounds like you may need some additional help. Have you spoken to your doctor about this? You may benefit from medication to deal with the panic and stress. You probably need to also make some changes to your daily routine to disassociate with triggers.
You're here, you have the desire to quit, so you're halfway there. You just need some reinforcement and a better plan of attack. I know that answer is very vague, but it's different for everyone. You need to identify the environments where you're having the most trouble, and come up with diversions.
As far as mornings, do you exercise? If you have the time, you may want to consider a stretching or exercise routine in the morning (I engufled martial arts and meditation when I quit and am in better shape now than I have ever been). Even if it's brief, it will help. Good luck!!!
