Not sure if you mean whether or not the blowing the smoke out of the nose makes you ill but I'm sure it can damage the nasal lining, and yes I've been getting heartburn from smoking too much

Smoking has been linked to an increase in sinus problems (I quit 1 1/2 weeks ago and I can already feel a great improvement in my sinuses!) and NHS direct (UK website that gives advice on health problems, they also have a hotline) recommends stopping smoking and using a humidifier/inhalator (one of those things that steam up and you can add vicks or olbas oil) as the only non-medication relief for sinus problems.
Mind you, it didn't stop me smoking for nearly 5 years while my sinus problems kept getting worse...
