List with neurological symptoms to present to specialist

Hi, since I've had a laminectomy last year in August my neurological symptoms list has become bigger and bigger. Some problems / symptoms went away, some came back, some are new. But overall the list gets longer.

I finally get a new MRI for my symptoms this week and later on a neurosurgeon will look at them and will meet with me.

Since I have almost a thousand questions and symptoms I want to write them down so I won't forget anything but I also want to keep lists with symptoms for future references. Also a list with my symptoms might be convenient for the neurosurgeon to read for an evaluation.

Do you keep lists of your symptoms? And what would be the best way to present a list with symptoms so a neurosurgeon / neurologist could also quickly read them and understand it?
