Hi , I am sorry to hear about your ongoing problems. I don't know much about the disability / insurance thing in the states. But I would advise you to make sure you get copies of all notes to do with your surgery and treatments. And certainly make use of your primary, and try to have him arrange for you to attend a pain management DR /clinic.
From a PM clinic you will have ongoing assessment and treatment.They are generally very thourough and keep copies of all of their notes as well. The PM doc can certainly help you with disability because of the close monitoring of your condition and progress. Which can only help with your disability claim.
