Disability Leave from Work - Questions

Has anyone needed to go on Disability due to thyroid disease? If so, how did you go about pursuing this? Did your company give you forms for your doctor to fill out?

Just adding that I'm talking about short-term (not permanent) disability.

I havenít had to leave work due to my hypo, however I know about short term and long term disability. You can get the forms from your HR department or if you know the insurance company that your company uses, you can get the forms from them. You will fill out some and there will be a place for your doctor to fill out. Some doctor offices charge a small fee to fill out disability papers. You should talk to your HR department about the coverage and they should be able to give you either a document or explain to you about the coverage percents and length of coverage options to you. If you havenít yet, you should also look into filling out FMLA papers for yourself. FMLA is the Family Medical Leave Act, it is a law that when someone has to take a lot of time (either all at once or spread out over time) they can not be fired for their time off. It is not there for the payment end, just the security end of not loosing your job. You should be able to get the FMLA papers from your HR department as well. Good luck to you.

Wendy, that was very helpful. I wouldn't have known to fill out FMLA forms if you hadn't mentioned it. I will speak to my HR contact person.

I'm still hoping that this leave won't be necessary, but today is my third straight sick day, and I'm running dangerously low on sick/personal days.
