I wouldn't add T4 as of yet. Many times it is too much T4 that results in the joint gripes. Call and see if the MD will set up the lab to draw your blood now. You know the standard TSH 3rd gen, Ft3, and Ft4. This way you have the results in hand when you see him next week. It will also see that your care keeps moving along in an efficient manner. To go in and see them get blood drawn and go in again in another week is a waste of their time and your money. If the MD is reasonable they will draw your blood while you are having symptoms. Best way to catch things when they are Wonky.
