Dr. David Hoffman at the Staten Island University Hospital. As busy as he is, Dr. Hoffman has been available to me by cell phone ANY time I have needed him (he once called back while at a conference in Austria, another time he called back while doing probono work in South America). He has a very conservative approach, which is great, but he also remains open to more agressive treatment, like arthroplasty or joint replacement, when necessary. He is the best of the best when it comes to TMJ surgery!
Dr. Gary Warburton at the University of Maryland Dental School in Baltimore. Sought him out for a second opinion on joint replacement and he has a lot of techniques and resources for non-surgical and/or post-surgical treatment all within his office or the UMD hospital right next to his building. Also, a very kind man with a very gentle manner.
Dr. Sylvan Mintz in Bethesda, Md. A former pediatric dentist, he is now a TMJ and sleep apnea specialist who treats both adults and kids. He's great with splints and works very well with other doctors, surgeons, physical therapists, etc. and will initiate a team approach. I highly recommend him if you are looking for a TMJ specialist in the D.C. area.

Call and make sure that they do Neuromuscular dentistry with Functional Jaw Orthodontics. They should also belong to the American Academy of craniofacial pain. (AAOCP).
