No-one! I have been searching for TMJ experts in my area and I found 4- they are all bioesthetic dentist - trained at OBI institute. Their belief from what I can gather is before any dental work is to be done, they make sure the tmj joints are happy, then proceed with stabilizing the bite to accomadate the joint.
That sounds like the approach being used by the dentist I am currently seeing. He did a lot of testing and measuring to determine the best position for the joints, and is going to redo some restorations because they are throwing my bite to one side. He showed me what he is going to do on models of my teeth and it was quite interesting. I could clearly see that one whole tooth is much higher than the others on that side.
