If the decongestant does not work a medical dr can prescribe meds that may help. I have found on occassion that bending my head down low or between my knees and then rasing it back up over and over a few times helped loosen it so it would come out on its own - but that was when I had fluid from a cold and did not have tmj - so if you are dizzy or have neck issues from the tmj sit down while trying this and don't "jerk" your head upright thinking it will jarr the fluid loose - its really the change in head position that helps. You could also try lying on your back on your bed and letting your head hang over the edge - the same concept of changing your head position would apply. Good Luck.
