Paul, second, third and even fourth opinions are not a bad idea. The doctors you could consider looking into are: another TMJ dentist, another Ortho, a Chiropractor (sometimes they can look at the overall picture and find what the source of the problem is beyond just the TMJ/TMD issue. find one who practices natural therapies as well, they will be able to see where you're out of alignment and not just do major manipulations to treat it. ask also if they know of a good dentist or ortho who could help you with your issue.)
You may need to just start over again and try and uncover all the layers of pain and where it's coming from. To do that it's good to ask around, find out what different doctors approaches are and ask them to recommend the right people to see. Try one thing at a time and work it backward by relieving your muscle pain, by going to PT, massage, or cranial sacral therapy. All of these methods can help in keeping your neck and jaw muscles from spasming, which is where the pain comes in.
And remember, this isn't cancer, heart disease or any of that which is causing your pain. It's just a joint and muscle issue. It's difficult to get through and debilitating, but with the right help, you'll be able to get through it and go on with your life. It's not worth giving up anything over.
