It sounds like you need to get your mandible repostioned. The problems you describe can be caused by the bite being off which in turn causes the jaw joints to sit in a compromised position, pressing nerve fibres, causing muscles to go into spasm and affecting the ears as well as a whole bunch of other symptoms.
Dizziness is a part of this picture and the best way to treat it is to deal with the cause.
I'm 4 months into wearing a Gelb appliance to bring my mandible down and forward and very quickly the episodes of dizziness / brain fog eased off and all the weird noises I was getting on opening closing stopped. I am still in pain but it has changed, its no longer diffuse facial pain but its now actually localised in the TMJ itself.
