Unhappy Help!--nightguard/splint questions

Hi all---I've recently been having trouble with my dentist designing an effective night guard. I have pretty bad tmj, which ones should I be wearing?
I couldn't tolerate the NTI, and I just tried a flat plane splint, but in certain positions, it feels like i"m only touching in 1 spot, --major problem for joints. My last splint was "worn in" perfect to my bite but with a little room to move on it, not real bumpy at all. IT was originally designed to be a 24/7 wearing splint, and then I only wore it at night. My joints are way worse now, so I"m finding nothing comfortable, (at least what my dentist has tried so far).
Any input would be great..J.

Maybe you need to see a specialist like I did. I got my splint about 15 years ago, so I'm not familiar with all the names or types. I just went to a prosthodontist, a specialist who makes prosthesis for people's mouths. He made the splint from a mold of my upper teeth. It fits snuggly, and that's how I like it. It keeps my teeth in alignment and prevents me from clenching at night. I hope you get it worked out.
