Unhappy TMJ face change

In september I had Zoom!whitening done at my dentist's office which was the biggest mistake of my life..
not only did it cause the unexplainable pain, it took of all of the enamel..
however I was in so much pain I didn't know how to handle it and I remember
I just bit down hard on the towel.. and I believe that's what caused my TMJ..
in October, my left lower jaw started to hurt bad and my cheek was swollen.. I went to the dentist and he just thought I had stress..
but the pain and swelling moved over to my bottom neck.. then over to my right side of my face.. my face swelled up like a puffy bread and my very back teeth hurt SO BAD that I almost wanted to die..
then I felt like my jaw was dropping down like a SQUARE and I can seriously tell the difference when I look at the mirror..
my neck is always swollen as well as my face, and others can notice it too.
so I took advils, and yesterday I went to the acupuncture and I don't know if it's working...

1) how can I make the swelling go away?

2) what do you think I should do?

3) I KNOW I don't grind my teeth at night, because I've never had this kind of problem UNTIL I went to the dentist's office.. do you think wearing splints will help at all?

4) my face is changing,, how can I stop it?

5) are splints made to go back to my normal jaw position?
or do they just help to MAINTAIN my CURRENT jaw position?
or do they CREATE a NEW jaw position?

6) I don't have the popping noise YET,, but I can fit 3 fingers.. is this a good thing?

7) the back teeth seem to be the ONLY teeth that are touching. I think my teeth alignment shifted somehow and I had braces two years ago..
if the problem seems to be my lower jaw moving back a bit,., what do I do?


8) my ears sometimes get plugged up as if I am on a plane and I don't have ear infection.. what do I do?

9) my acupuncture doctor said it will be cured within few sessions.. can I trust him? does acupuncuture actually work?

10) is your face changing too? if so, how?


PLEASE Lord,, help me........ it's my senior year.. and I can't do anything..

Wow---everything you just said is a mirror image of me 18yrs. ago when I had braces done wrong on me, and my wisdom teeth taken out. In my unprofessional opinion, the reason you are only touching on back teeth could be: Muscular Trauma from having your mouth wide open for a long period of time
Or the dentist actually triggered an underlying predisposition of tmjd, and worst case dislocated on of your discs.
You need to get to a good tmjd specialist for a complete exam, x-rays of your joints, etc. to see what the problem is. Continue taking advil, don't go over the recommended dose. Also, use a hot water bottle on your face as much as possible to relax the muscles. I think my specialist knows of a doctor near you if you want me to email them, I'd be happy to. Where exactly are you located? please let me know before I do so.
Also, to answer your question about treatment. If you went to my tmjd specialist, for example, they would take molds and examine you bite right away, do tomograms, (x-rays of joints) and make a soft splint you could wear to take some of the load off your joints and bring your jaw down and forward a bit. They may even give you Flexeril for muscular spasms for the time being. After the initial testing would be done, (computer scans, neuromuscular testing, etc. ) they would make a hard splint for you to actually reposition your jaw---If need be. This would ONLY be determined by the tests they run, and your examination. The earlier you get in, the faster you can nip this thing in the bud, so to speak. I'm in treatment right now, and I've had tmjd for 20yrs. , so if you have any questions, please ask away.
Let me know where you live, and I can see if I can find a good specialist for you. Look on the ICCMO website, and click on members. See who is in your area, and call and make sure that they practice neuromuscular dentistry, and make sure that they are also a Functional Jaw Orthodontist. In my opinion, these are the people who know what the heck they are doing. Look on the old post to Silverwings that I bumped up yesterday for Hwoods, it lists all the testing and quesitons you should ask and have done at an exam or consult.

PS---Whatever you do, don't waste your time at the dentist who told you that it's just from stress. That's total crap----They are just trying to push this off on something you did....it sounds like the typical situation of a dentist screwing up and trying to blame everything on the patient who is in terrible distress. OOOO, that makes me furious!

You poor thing, what a miserable situation. The advice you got from Jmitchell is great. She is so supportive and helps everyone here as much as possible and I think all her thoughts are right on the money. Don't go back to this dentist, ever. What a jerk. Follow the previous advice and let us know how you are doing. feel better.

to see some wonderful people like you guys,, who actually understand what I am going through.. who genuinely care about the suffering.. thank GOD I've found this thread this morning before I went to school.. I just wanted any last hopes..

honestly these past days I've realized I am becoming weak & hopeless as my pain continues to grow.. I don't think I have that oomph or motivation anymore unlike how I used to have.. and I know I should think in postive ways as all of my family and friends tell me, but when my pain dominates my brain, there's nothing much to think about.. except to end the pain.. I'm just a senior at a highschool and when I had THE pain of my life like a week ago, I thought to end my life to end my pain.. because I didn't know what to do..

I thought I was going to faint at school today because I think it's spreading to my head & neck,, causing all of unbearable pain.. although I am not sure if the acupuncture has made it worse.. but for now, I don't think I'm planning the second session..

does the splint actually work and improve TMJ?
what is the cost and how long do you have to wear them?
how do doctors know my previous jaw alignment?
if I get the splints, would my face go back to normal? like the chin & cheeks & square jaws will be roundy?
do you know for sure it will work?

tmrots; thank you so much for your words.. I don't think I should go back to my dentist either.. although I want to get a refund on that terrible experience.. It's very nice to have people like you and I'll keep you updated!

today I've noticed when I was going upstairs / jumping / running / swallowing / I heard clicking sounds in my ears... what can possibly fix this?
and my jaw/ lower teeth are moved backwards so I can feel the gap between the top teeth and bottom. no wonder why my retainer was tight.. why is this?


will splints cure all of this?
I am more calm today than I was yesterday.
please help..

Hi there--Your teeth are probably not meeting correctly because of the muscular spasms you are having. The clicking and sounds in your ears are due to the position the mandible is in right now. (the bone from your jaw is only mm.'s away from your ear drum.) Please don't worry about that. The most important thing right now for you is to get an exam by a tmj specialist and see if they can stop the spasms. Do you still have your braces on? Are you still under the care of your orthodontist? Shame on them for not taking care of this----and referring you to the right help. Please think of going to one of the doctors that I posted for you to get an evaluation. Something when your teeth were whitened triggered tmjd symptoms, and you need to calm those muscles down.


What did your parents say about the situation? Please tell them that you need to get in asap to get a consult so they can calm the spasms down. If they have any questions, I will be checking emails tomorrow morn. I'm glad that you are calmer today. Keep using a heating pad or hot water bottle, and advil as directed for pain. Hang in there----J.

I'm tearing rightnow..

I was done with my braces.. I got them off 2 years ago..
ever since I got my retainer, I've been wearing it everyday so my teeth never moved back.

but after I had jaw pain about 3 weeks ago, I felt like bottom retainer is tightened when taking it off and on.
there's nothing wrong with retainer because it's not adjusted and it maintains my correct alignment.. so I was very curious because there was no reason for my teeth to move back because I've been wearing it everday. and it was the only bottom one that's tightened..

yesterday I felt the gap between my top and bottom teeth.. and I've noticed overbite was created.
today, just now, when I close my mouth, the bottom teeth is almost half covered with the top.. the overbite is developing and it's getting worse..
and the clicking sound it very noticeable..

it's so sad how dentists/ orthodontists/ TMJ specialist offices are closed on fridays.. great now my overbite is going to get WORSE..

what do you think I should do..

Just from the info. you have given me, what I think happened was the same thing when my ortho was done years back. The new bite they created for you brought your lower jaw too far back, causing spasms to start, triggered by the teeth whitening episode. When jaw muscles are in so much spasm, it is going to shift your bite a little bit, think about that, it's pulling your lower mandible totally out of alignment, thus creating a feeling like you have no real resting place or "bite" for your teeth. It's extremely uncomfortable , to say the least. You need to have your parents call the tmjd specialist on Mon, and get you in to be looked at. The longer you wait on this, the harder everything is going to be to treat. They maybe in on Sat., you might want to check. In case you end up locking closed, please do this asap---gag yourself really hard, the position it puts your lower jaw in will hopefully recapture the disc, and pop in back in place. You will know right away if It works. When all of this started happening to me years back, this trick saved my life many times, LOL. IT's not fun, but it sure works.
I will be checking in later on today if you have anymore questions. Please have your parents call and see if anyone of those docs are in today.

If the swelling is being caused by hyperactive facial musles you need to get your bite repositioned

Appliances/splints/bite plates can be very helpful. However it does depend on the direction the disc is displaced and the extent to which it is displaced. If its displaced to the front then there is something like a 70% chance you can get it back to its correct anatomical position. Mine was 40% out and half sideways and half to the front so in the end I had to have surgery.

See question 1. My face went absolutely weird at one point before I got diagnosed. It became wider than long. All the grinding caused my cheek muscles to bulk out. At the time I just though I was getting fat but now I can see my cheekbones again

Splints will create a new jaw position. I think most people on this board have jaws that are set too far up and back which not only leads to disc displacement but also means you are closing down onto nerve fibres because you shock absorber is MIA

3 fingers is a good opening. The more to the fromt the disc displaces the more opening will become limited, if there is a side ways displacement opening won't be affected as much

Get a splint to bring you down and forward. There seems to be two types, a pivot and a repositioning. The pivot only has one point of contact on both sides and is designed to get the disc back in position. The repositioning appliance has an indexed occlusion so you close into grooves which holds your jaw in a fixed position. I wore a pivot to start with and now am on a repo which feels more normal

Look at some anatomy drawings, the ear canal is right next to the TMJ. If your jaw is too far up and back then there will be an impact on the ears

Be wary. All these alternative medicine practitioners need the business and will make promises they cannot deliver on. He may be able to treat the pain in isolation but if the cause is not addressed then you won't solve the problem. I prefer to deal with facts and if you go to a good serious TMD specialist they will carry out proper diagnostics which will flag up whats wrong. You need a whole bunch of x-rays and an MRI scan to image the soft tissues

10) is your face changing too? if so, how?
With the repo splint my face is back to how it used to be

so today I went to see my orthodontist first, because it seems like the professor he recommeded have a poor communication with everyone..

my orthodontist thinks I'm okay and that TMJ will naturally cure by itself and I should stop reading from the internet. I specifcally told my ears click everytime I eat but he said they can click without TMJ. We took an x-ray to see if wisdom teeth were also part of this and found out NO they weren't. He just wants me to keep taking the advils to reduce the swelling but he knows it's only for temporary. my mom thinks it's "All in my head" and she just believes what 'the professionals' say. eveytime she says that, I just want to kill myself in front of her eyes. I also told my doctor my jaw feels it's moving back and he had me open and close my jaw and had his hands touching them but he thinks I'm fine.

the orthodontist recommended two best TMJ doctors around here and it seems like we can't get an easy hold of them. he says he will contact them for me because it's keep delaying, that means I would have to wait LONGER on this and never know when I will get to meet them. this is just getting so frustrated and my parents are helpless..
I still love them but they just want me to wait until we can get a hold of the professor.
WHAT IF he says I'm fine? and I still have all these symptoms

since three days ago now I have muscles twitching all over my body. my parents think it's because I didn't eat much these days. GREAT.

Oh my----You have your hands full I think. need to know that if you get the right help, especially since you've just started with symptoms, that your case should be easier to treat than someone who has had it for years and years. Second, please run, don't walk from your orthodontist that said your tmjd will magically just "go away on it's own". THat's ridiculous, and shame on him that he is saying that to you and your parents. He really sounds like a piece of work. You need to get to a tmjd specialist to get evaluated. Did you give your parents the two dentist's names i researched for you? What did they say? Good luck with everything, if you want to give me the names of the docs that you are waiting on so I can look at their credentials, I'll be happy to do that. Hang in there,----J.

I told my mom that you recommended these two doctors; Dr. Simmons & Faber for me but she wants to go to the UW professor first because ortho & parents think professors are the best? .. His name is Dr. Edward Truelove.. the guy that my ortho recommended. SUPRISNGLY, my ortho recommended another guy at the same time, his name is Dr. Jason Pehling or something. so I searched Edward Truelove on google and the first link I believe, gave me this thread and someone asked for TMJ specialists in WA and those two names came up. So I guess they are kind of.. dependable.

and 5 mins ago we FINALLY received a phone call from UW Dentist office. but we have to call them tomorrow morning at 8 to make the appointment for DAY AFTER. so I'm probably going to see Truelove on Wednesday.
but as I was searching, this guy also had TMJ since the age of 8, went to Truelove but he said he was fine.

so now I'm hesitating. but like everyone says, I should probably try multiple specialists because I have symptoms getting worse. so I will try Dr. Simmons too.
rightnow my symptoms are: lymph notes getting puffier and blocking my airway, my muscles twitching all over my body, difficult to swallow, clicking in ears everytime I swallow, face swollen, only about two fingers can fit in my mouth.. etc.
and all I've done so far were: going to the dentist, acupuncture, warm pads, advils, visitng my ortho & had xray..
YEAH. and I such have a weak heart to think the dumb way; killing myself. but it's really diffcult for me especially my mom believes the doctors and that it's ALL IN MY BRAin.

you should be my mom.

Hi again,

The main ones that a good tmjd specialist belong to are:
1. The AMerican Academy of craniofacial pain
2. the American Academy of orofacial pain
3. The American Academy association for Functional Orthodontics
Also, they should be using neuromuscular dentistry as well.

I've never heard of this doctor, doesn't mean he isn't good, but he's hard to find info. on. You can always get another consult by Dr. Simmons as well. I have a feeling that their treatment styles are very different though. I'm sorry that everyone is telling you to not read on the internet. That is the one source of info. that has literally saved my life this year. I get so angry when dentists will say, "you know too much--" , or "don't listen to anyone on the internet, they are getting kickbacks or don't know anything..." I guess you just need to go with your gut, and be cautious, say what you feel, and don't back down when someone is telling you it's all in your head. I assure them , it's very real. Nineteen years real for me. Anyways, good luck, I'll get off my soapbox again. Try to stay positive and as calm as you can. J.

I will consider those categories when selecting the specialist.
if I was living your area then I'd probably go to your doctor, because from the previous descriptions of the treatment you described probably work for you. and it'd work for me too. but it's really difficult for me to choose the right doctor who does the same way as yours does.. does your doctor know if any of tmj specialist in my area who does the similar treatment?

my biggest concern rightnow is the movement of my jaw. everyday I can feel my lower teeth are out of alignment, like it's moving closer to my neck.. and today my ortho checked it and he said the alignment is fine. I really don't know what to do about that one because I'm the one who can feel it, and he is a very nice guy.

I am guessing that I need a splint therapy to move my jaw forward a bit and create a new position for my jaw. but at the same time, I am VERY afraid if the professor says I'm "fine". because then, my parents will believe what he says.. but I will not stop to convince them to see another specialist.

the symptoms are really starting to affect my neck. my neck feels stiff now like a tree. wow I have ear pain too!
I'm trying to be calm as I can be. I want to follow every advice you give me because you really understand what I'm going through.. but I'd probably freak out if one day there's no more jmitchell on the thread. it's like losing a dependable teacher! thanks for everything.
