A lot of times tmj symptoms are caused by muscles. I would always recommend getting checked out by the doctor and dentist first, as you've done. Then, if you can afford to pay for it out of pocket, try to find someone who does intraoral tmj massage. The cracking sounds can be made by the lateral pterygoid muscle. The ear symptoms can come from the medial pterygoid muscle. They need to be massaged from the inside. The practitioner wears surgical type gloves while they do this. I suffered for two years before finding certified neuromuscular therapy. I had to drive an hour and pay a lot of money but after three visits my symptoms are about 90% better. And what a relief it was to find out that the symptoms were just muscles, and not something bad wrong with the joint or the ear.
