Asymptomatic Microscopic Hematuria

Hi everyone,

I discovered microscopic hematuria in my urine about 5 years ago in 2012, This information was received during a pre work physical. At that time, I also had some blood work done. My bilirubin was measured at 2.3 mg/dL, other results were fine (including AST, ALT, white and red blood cell counts etc.).

I did not have any follow up testing done to look into the cause of this.

Now, it is 2017, I get another pre employment physical, I find that I have microscopic hematuria and my bilirubin is at 1.4 mg/dL in my blood test.

I realize that this was discovered before, now I am older, and feel a bit more responsible and concerned about my health than I once was, so I followed up.

First with some blood tests, creatinine, GFR, and BUN results are normal.

Next with Non-Gyn cytology. Results state negative for malignant cells, Bland urothelial cells present in a background of squamous cells and mixed inflamatory cells.

Next I had a CT of my abdomen and pelvis with and without contrast.

Multiple scattered subcentimeter hepatic hypodensities were observed. The results say they are too small to be characterized, but are likely "presenting cysts or biliary hamartomas".

Other results of the CT are normal (lower chest/lungs, gall bladder, pancreus, spleen, adrenal, gastrointestinal, urinary, bones, soft tissue etc.)

So now I have these questions,

With the inflammatory cells in my cytology, my first thought was, maybe a UTI. But no abnormal bacteria indicative of a UTI was discovered (according to my doctor but she did not specify the species of bacteria that was present, only that the bacteria was not indicative of a UTI).

Could I have a UTI but bacteria not be present in my urine?

In regards to the CT scan, subcentimeter hepatic hypodensities present. Could this be early signs of cancer, even if malignant cells were not discovered during the cytology?

If I had kidney stones or a calcium buildup of a sort in my body, could that cause the microscopic hematuria, while simultaneously be non observable in the CT?

Even if the hypodensities were cysts, could one day they become cancerous in the same way that a mole on the outside of the skin can? And how many people have these cysts? Do they typically just go away, and if so, how long does it take for them to go away, or do they stay indefinitely?

Lastly, if it were "biliary hamartomas", would that be something to be concerned about?

Has anyone ever experienced anything like this? Asymptomatic microscopic hematuria, and not be able to figure out the cause?

I have this turn up on my yearly physical too. Microscopic blood in urine. Been going on for 5 years now, ever since I had a kidney stone episode. I went through tests such as yours even a cystoscopy, ct scan xray, it turns up nothing wrong. My blood work is all good. They say some people just have this with no cause. The more I read on these boards I see more and more people in the same club. Kidney stones can be a cause as well. What does your doctor or urologist say? The cystoscopy test they use a camera to look in your bladder and kidneys, to look for cysts or stones Try and not be so alarmed about this. I was when all this went on and now I forget about it till I get the yearly urine test, and say oh yeah that is still there..... Good luck to you and hope it is nothing as mine turned out to be.

I've had microscopic blood in my urine since January. Had all tests and no cause found for it either. Only other times I've had blood in my urine was with UTI and after kidney stone removal.

Just giving an update,

So, I had a CT scan, multiple urine analyses and cytology.

Nothing showed up in testing.

I have a follow up in a few months for some blood work and cytology. I imagine, if the microscopic hematuria is still present (which I have no reason to doubt that it will be), I will get the cystoscopy.

Basically, my urologist mentioned that it is somewhat common for people with AMH to undergo testing and to have nothing found. Something like 1/3 of his patients leave without discovery of the cause, and something like <2% of his patients ever returned at a later date with an issue after being discharged.

After my next follow up in a few months, I believe I will return in another year or so for another checkup.
