Welcome to the board. I am glad you found this resource. Four years is a long time to be dismissed by Drs and have all these symptoms! First of all, I'd like to mention a few things that could be possibilities, but obviously needs checked out by someone who knows about these things. You are describing typical urethritis, orchitis or possible epididymitis, and possible cystitis. All of these things can relate to a past or present chlamydia infection. Has anyone ever done a chlamydia culture from the urethra? The infection, if not diagnosed and treated, can cause an inflammatory reaction called Reiter's syndrome (reactive arthritis), which includes inflammatory urethritis and inflammation of other areas, including eyes, joints, sacroiliac joints, and heel pain. A good series of blood tests for that would be anti-chlamydia antibodies, IGG and IGM, to help differentiate current from past infection, a sed rate and CRP, to check for inflammation, and if desired, an HLA B27. Not necessary if antibodies are both negative.
The other thing that comes to mind is Behcet's, a rare inflammatory condition that also causes urethritis and ulcer-like sores on the genitalia, as well as a host of other symptoms. There is no good test for that unfortunately, and a rheumatologist would be the likely Dr to know about Reiter's and Behcet's. Good luck. I hope you get some answers and help soon. Be persistent, and forget the Dr. who has lost interest. After you get a diagnosis you can inform him of what it is you have been complaining about for 4 years.
