mid-cycle bleeding

I'm looking for some ideas (and possible reassurance) since my dr. can't get me in for an ultrasound until next week.

I've been on the pill for a long time (for b/c but also because without it my periods were quite erratic). I have never had mid-cycle bleeding while on pill (I'm 46). I did take one pill late this month at the beginning of the packet, but made it up the next morning--which has happened before. Also on Z-pack for bronchitis.

On sunday i started having some spotting, which has proceeded into today and progressively gotten to a heavier flow (not ridiculously heavy, but definitely heavy than my periods generally are and with occasional clotting).

Called dr. and they are going to do ultrasound on Tuesday. Any ideas on what could be causing this? I don't want to freak out (i have GI issues as well, which often mimic many of the symptoms of ovarian cancer--gas, bloating, etc--so of course, I'm kind of freaked out).

any ideas/thoughts/ suggestions would be appreciated.
