When will I start to see cancer changes

My sister who is 54 was just diagnosed (5-100) with metastatic breast cancer that has moved to her lungs. The doctor said it is inoperable and has put her on oral medication to control the growth of the tumor. The doctor said she probably will not live to be an old lady. Our family realizes the possible outcome of this horrible disease. My question is when will we start to see the physical effects of this cancer? You would never in a million years know my sister is sick because she is still so active. She is not able to work because sitting causes her the most discomfort. She does get tired but other than that there are no signs that she is sick. She is still golfing, hiking and riding her bike. Everyone is praying and trying to be very positive. I feel I needed to prepare myself for what lies ahead. Is there anyone out there who has had a friend or love one be like the energizer bunny and then start to fade? How many months after being diagnosed did you start to see the effects of cancer? I do realize that she could survive for a long time and of course that's what we are all praying for. But I'm scared because I know the reality of the situation and I feel I need to have an idea of what to look for and expect.
