puffiness or bags under eyes

Hi,
I noticed puffiness under my eyes about six months ago. I'm not sure, but I think it has looked better at certain times, and it seems to be worse under my right eye.
How do I know if it is water retension or fat? It's not super bad, but enough to make me look tired out.
I don't go to doctors often (and usually don't feel confident about what they say anyway), and I'm not sure if I should speak with a cosmetic surgeon or a physician.
Anyone know how to tell what the problem is?
