Certainly it could be a failed root canal. Root canals have a 5-10% failure rate depending on who did the work. Did your dentist take a digital x-ray? If he did, that would probably not reveal much of anything. The best thing for you would be to go to an endodontist and have him/her check it out. They use higher dose x-rays which reveal more as well as checking out your symptoms.
