I know this message is a little late but I wanted to relate my experience. Are you sure that you need this surgery? Have you checked out other cosmetic options? When I was a teenager a orthodontist (I was referred to him about one crooked back molar) recommended that I get surgery to on my jaws to "align" them. Cut the jawbones to move the lower one forward and then have me wear a head gear at night for the rest of my life! No joking. Let me add that my teeth were perfectly straight except for an overbite, and my bite was not causing any kind of problems. My parents fortunately were upset but smart enough to realize this was not really needed (my dentist thought so too). And I didn't have it done.

You might want to search around and check out other options. Did the braces straighten out your teeth but you weren't wearing your retainer so then they relapsed? Maybe having braces again might help; perhaps not enough space in your mouth for all your teeth and so they go crooked, and having one tooth pulled might help. Or check out some other cosmetic options such as implants or removable front teeth--they can do real wonders nowadays. I'd really get a second opinon--breaking your jaws seems drastic. Good luck.
