If your insurance covers it, I would ask your dentist for a referral to an endodontist, or root canal specialist.
Antibiotics should begin to provide relief within 48 hours. Could be that specific antibiotic isnt working, you may need to switch.
Sometimes teeth can be damaged from a deep filling or a crown. The nerves become very irritated, sometimes they heal and other times the tooth begins to die. This could lead to a root canal.
I would ask for a referral, or get a second opinion from another dentist.
