Maybe you should see your doctor. Often times, sinus problems can lead to chronic bad breath. Also, if you suffer from allergies, this can cause the sinus problems. If you breathe through your mouth, this dries out the oral cavity, and allows the bacteria to flourish, making the smell worse. Try breathing through your nose if possible. Some people seem to have a dry mouth. If you think this applies to you, there are oral supplements, such as rinses, that can help replenish the moisture in your mouth.
Have you had a cleaning recently? If not, try this too. From my experience, people think they are sometimes doing a better job than what they are. You'd be amazed at the amount of plaque and food I remove from a 'clean' mouth.
