Now about your root tip, saying that there is still a root tip is rather vague. There are two roots on tooth #19. When the apico ectomy and the root amputation were done, the doctor could have been treating only one of the roots,(unlikely). But since the tooth was still retained in the mouth it was being held in there by something... which would be some of the root. So when the tooth was extracted some of that root could have broken off as the tooth was being taken out, and so it would be referred to as a root tip. Also during the apico ectomy or the amputation the Dr. could have left some of the tooth behind then. Last "root tip" could mean that there is a large piece of the root or a very small chip of tooth that is still present.
