It sounds like a touch of schizophrenia. Of course, all the psychiatric illnesses have considerable overlap, so that's probably not an accurate diagnosis. But some of the things you describe are related to schizophrenia-like behaviors. You are obviously not a full blown schizophrenic, but there are some subtle components present.

A small dose of Risperdal (1mg to 3mg) or a small dose of Zyprexa (2.5mg to 5mg) could fix this. These are antipsychotics. At these low doses they are relatively mild. I prefer however to call them dopamine reducers. More user friendly. The talking in your head is often caused by elevated dopamine, and a dopamine reducer/blocker can correct that chemistry.

Sometimes the SSRIs can work too. Each of them have subtle but distinct differences. In your case, Paxil or Zoloft or Celexa may be more appropriate than Prozac or Effexor. But if I were in your place I would probably start with Risperdal for 2 to 4 weeks first and see how that goes. If not helpful, than at 4 weeks I move onto the other choices.
