Hello Barbie,
I am unsure if this is what you're looking for but here goes.
I created my own chart on an Excel spreadsheet. I record my BP readings manually every morning and enter these regularly [ie, every month] onto the spreadsheet. One month = 1 page. The fields I record are; histolic, diastolic, pulse rate, date and time. I've set the page up to calculate the maximum, minimum, range, average and standard deviation for each field. Yep, it may appear to be an overkill but after approximately 30 years of untreated hypertension, I am now able to bombard my GP with a daily record of my B/pressure. Previous GP's had often remarked that my blood pressure was "a little high", had done nothing in the way of treating my condition. I now believe that they had attributed this to a phenomenon often referred to as the "white coat effect".
My objective is to avoid suffering another brain haemorrhage the effects of which have changed my life considerably. If this is of any interest, I would be happy to email you copies of my typical spreadsheets. It will be quite easy to customize them or your own use if you are reasonably familiar with Excel.
On the other hand I could scan you with a scanned copy of a simple data recording sheet if this is more suitable.
