Astigmatism is usually caused by the front surface of the eye being non-spherical, like the side of a football, compared to the side of a basketball. The axis is only a measurment of which way the "football" shape is aligned, ranging from horizontal to vertical, back to horizontal (0 to 180 degrees).
The Cylinder power is what measures the degree of severity. It will go in .25 steps, ranging from very mild, at -.25, to moderate at around -2.00, to severe, at around -4.00, and sometimes very severe, up to and over -8.00 (or +.25 to +8.00, depending on how your eye doctor chooses to write your Rx, it can be transposed to + or - signs.)
