I also wake up frequently during the night with "dead" arms, and then the tingling sets in. I have not experienced it in my legs, but often I get uncontrollable shudders in them that goes on for hours sometimes. Nobody seems to have an answer to this problem, but at least my doctors recognize that many FMS sufferers experience this.
Personally, I use a foam roll to support my neck during the night and it has helped tremendously. My pain is worse in my neck and shoulders, and I think that because I always have the perpetual "shrug" and tension, it interferes with the circulation and nerve response to my arms.

I am also in California, and with Kaiser. I have a fantastic (young) doctor who is very well informed and treats many Fibromyalgia patients. I also am working with a very knowledgeable Rheumatologist at Kaiser, who heads up the Fibromyalgia Workshop I attend at Kaiser.

Good luck finding a new doctor... it's a hassle, but really worth it when you find a good one.
