A culture is a laboratory procedure to detect the presence of micro-organisms and to identify which specific ones they are. In the case of herpes, they're looking for the herpes simplex virus (HSV). The doctor basically takes something like a large q-tip and using the cotton tip, swabs the affected area to collect the micro-organisms so that they can be analyzed (it doesn't hurt). The "q-tip" is then placed in a test tube and sent to the lab where they "grow" the organism that's on the cotton tip and then do tests on it to determine what it is. I hope that explains it well enough.
