Empire Blue Cross/Blue Shield has many different plans. We belong to Empire Blue Choice PPO and did not need a preapproval, and had no problem with coverage for 7 weeks of IV. It may depend on the Empire Plan that you have. Visit some of the other sites on Lyme for more info on fighting insurance companies.
