Nephew has several problems, but no diagnoses, can anyone help?

My nephew is 11 yrs. old and has been in so much trouble since he was 2 yrs. old. He acts out in rages, he can't learn in school. He can't read which might be a problem all in it's self. But he is very destructive. Tears up his room and his sisters room and possesions. He has flushed live fish down the toilet. He has shot animals w/ B.B. guns and he also shot a little boy in the side w/ a b.b. gun. He uses foul language. He goes to neighbors houses and destroys their property. He has no conscience for what he has done, and does not apologize. My sister is going crazy because they can't get him a diagnosis and no meds seem to help him. They are leaning towards BiPolar disorder, but from what I've read about him, he does not seem to have the symptoms for that. He also has admitted to having a little man w/ him at all times that tells him to harm himself and keeps him from doing good things and encourages him to do bad things. This sounds like Pschyzofrenic (SP?) but the other symptoms does not go along w/ it. He has since denied having this man w/ him or hearing voices, but he has been caught several times talking to himself in deep converstations. He has become violent w/ my mother, his grandmother, and he cusses people out, strangers or otherwise. If anyone knows anything about these symptoms, or has any advice on other websites... ANYTHING.... Please, please let me know. My sister's health is failing, their marriage is failing all because of this. It's a bad situation. He has recently been placed in an In-House treatment facility and been placed on lithium and other drugs. He was placed their by the courts, not the family.
