Current treatments include:
1. Racz procedure with Wydase and steroid
2. Forced epidural steroid injections (transforaminal or interlaminar)
3. Catheter directed forced epidural steroid injections
4. Epiduroscopy using the scope to lyse scar tissue
In the near future I will be adding Holmium laser epiduroscopy which has shown great promise.
The Racz is currently probably the best of the group available.
Epiduroscopy in a direct comparison, has less efficacy compared with the Racz.
The others depend on being able to forcefully inject under pressure through scar tissue which usually isnt possible and can have severe side effects due to increase in the spinal cord pressure.
