I've had 2 kids..both by c sections. They'll insert it once you've gotten your spinal or epidural, it's a long skinny tube, they'll insert it, fill a tiny balloon with water so it doesn't slip out of you. Then in 12-18 hrs {i had mine in for different lengths of times with each c section}they'll remove the water from the tiny balloon and remove the tube. A little uncomfy but definitly not painful. The only thing that bothered me was trying to move around with it on. With my first they made me get up after only a few hrs, with my second i got to spend the entire day in bed with the nurses doing everything for me so it wasn't a problem the second time around.
