Assuming she's taking proper care of her first child and she's not drinking or doing drugs durring the pregnancy she will automaticly get custody, plus you said she's making an effort to get her life in order i see no reason for her not to have custody. Besides unless the child was in immediat danger custody would be decided in court. To make it offical she does need to go to court though, nothing really happens if she doesn't, but it's best to get those things out of the way asap (if the father doesn't deny paternity and doesn't want full custody it is a very simple process), she should also file for child support at the same time.
Chances are he brought up the custody thing to scare her, or he's scared he'll end up with the baby. They have no reasons to take the baby from her so she shouldn't be worried about that. She should talk to a lawyer (she should be able to get one through legal aid if you have that in your state/country) and see exactly what her rights are.
