Did anyone else's doctor put the I.V. in the wrong place?

My father told me that my doctor put my I.V. in the wrong place in my hand, and it is because of this this I developed a scar/hole in my hand (that still hasn't healed and prevents me from walking correctly and using my left hand). I am just so glad that after 10 months, it is almost healed; but I still have to get a skin graft for it to be the right color and I can't get the skin graft now because I am on coumadin (that's what I think I've been told). I hope that no one else had a doctor like mine (who, I guess didn't learn anything from his 7 or 8 years in college)
