tenderness in throat...thyroid?

i had a thyroid test in june because i was having a lump feeling in my throat..Tests came back normal..Now its gotten so bad. My neck is so tender to the touch, ihave a hard time swallowing and shortness of breath..I know anxiety plays a part in this but i know something is not right! i also have some other thyroid syptoms. My question is, could this possibly be a goiter or nodule i cannot feel because it is deep donw? should i request a thyroid scan..thyroid problems run in my family! and i also had a baby 9 months ago? thank you!!
