Hi. Sounds like you have quite a bit of trouble there, my friend. Although my TMJ isn't as bad
as yours, I'd still like to let you know what has helped me.
There are times when it can affect my speech, at any given moment, or
feel like my jaw might go out of place (although it never has actually done that).

I've been seeing a good chiropractor. All he does is have me sit in a chair. He stands
behind the chair, has me open my mouth wide, and then as I close it he gently but firmly
pushes the lower jaw into place. I've also tried concentrating more on whether or not I'm
clenching my teeth out of sheer stress. By taking note of this more it seems, to a small
extent, to help ease a bit of discomfort.
