    Question New GERD Symptoms ....

    I have had Upper GI Series done in April/05 which showed a Hiatus Hernia and Reflux. At that time I was bothered with swallowing like over a lump,chronic cough(especially at nights and for years-post nasal drip that I have could be responsible for this).I also had some stomach pain in the upper left under ribcage. I also had an abdomonal ultrasound which was okay.I saw the ENT and he said, " Anxiety!" I was put on Pantoloc(PPI) at the time of the stomach pain and also given an antibiotic for suspected H Pylori. The Dr. never did give me the results of the blood tests done at that time. He kept saying it took so long for results to come back.

    During May of this year I continued to have the swallowing problems (like swallowing over something,some dysphagia with dry foods and once in a while with liquids). I also started having a burning tongue, difficulty breathing(like shortness of breath and sort of a burning in the back of my throat),Night time and morning feelings like something on my chest keeping me from breathing well,( heaviness).

    I also have a lot of mucus in the back of my throat, and a sore tight feeling in my throat.I am hoarse in the morning for quite some time.

    I have never really had anything come back up into my throat that I have noticed but it does feel like the burning sensation moves up my throat. Tis is especially bad at night. I have difficulty sleeping because of the breathing , swallowing and throat symptoms.

    I have just changed family Drs. during the past month which may not be great because of lack of continuity but felt that I wasn't being heard about various things but that is a different story.

    I have been put on Pantoloc this week and have been told to see what happens in 2 weeks . Then we will likely do the endoscopy which my old Dr, hesitated to do a while back because he thought it was quite invasive. I do want to know though if there may have been damage caused bt the burning feeling I have.

    My neck is also sore and my shoulders.I have had jaw discomfort and ear pain with nothing ever showing in my ear. I may have some TMJ . I also have severe osteroarthritis is my neck and clavicle area which may be causing some of the symptoms.

    Have any of you had similar symptoms? Some that came later and decided to stay? What are your thoughts? After reading the board I really wanted some thoughts from others. Biya

    I too have alot of your symptoms. Rearely have heatburn. My biggest complaint is the awful lump in the throat with constant mucus. My neck hurts all the time and shoulders too. It always feels as if my neck is swollen, but its not. I had a scope done and the ent said it was lpr. Gave me nexium. I took it for 3 months and actually felt better after a week. Then Bam, I felt 10 times worse. My gp said nexium can make your neck feel awful, so he switched me to prevacid. Took that for 3 weeks with no improvement at all. So now I stopped taking everything. I'm starting to think its tmj or such. I just don't know what to do anymore. The stomach meds actually made everything worse. Anyone with suggestions?

    Note to Biyak 123

    Sure sounds like you have LPR. I have nearly all your symptoms including the hernia. Hope the PPI's work along with any dietary or lifestyle changes. If the sphincter is too loose it is unlikely that these changes will address it. The burning in throat and mouth is acid reflux. It takes less reflux to cause damage with LPR than heartburn with GERD.

    Note to Patti23

    It could be that you are one of the unfortunate few for whom PPI's do not offer any real relief. I would find out just how well your sphincter is actually
    working. This should tell you whether or not continuing with meds is is a waste of time.

    LPR is laryngopharyngeal reflux. It is when the acid goes into the throat and mouth area. All the symptoms Biyak spoke about are representative of LPR. Some people suffering with acid reflux have GERD symptoms, typically chest pain, chest burning, etc. while some have LPR symptoms, typically sore throat, swollen feeling throat, lump feeling in throat, feeling need to constantly clear throat, feeling like acid is dripping in your throat and secondary lpr symptoms of ear pain, burning mouth, burning nose, etc. A person can also have GERD and LPR symptoms. If a person is going to respond to ppis, then often 1 ppi / day is enough for GERD symptoms. However, for LPR, often the dosage has to be increased to 2 ppi/day and usually for at least 6 month in order to find relief. This is by no means written in stone as everyone is differnt but it seems to be what many say they experience. An endoscopy by a GI is the best way to check for damage to the esophogus while an ENT will scope the nose and throat area. I, unfortunatley, suffer with both LPR and GERD and am taking 2 Nexium/day with an occassion pepcid ac for really bad days. I also sleep inclined and try to watch what I eat and drink. Some days are better than others. I am planning on taking the necessary test to see if I am a candidate for either the surgery of one of the other less invasive proceedures.     Note to MJCM123,

    Sorry to hear you are still suffering. Have a look at the post by scooteraw as I think it sums up where a lot of us are at.I had my manometry test and 24 hr PH monitoring week before last.....waiting for results which I pray will shed some light on this. Keep you posted.

    Hi Ginger. Thanks for referring me to scooteraw's thread. Definitely post your test results. Some days I think I can get a handle on this and then others I feel like they couldn't offer me surgery fast enough.

    I have another thread that I don't know if you have read. I was at the ER today with my shortness of breath and difficulty breathing. They did the Blood tests, ECG ,a chest X-Ray and had me hooked up to an oxygen nebulizer. Every thing came back normal. They asked about a history of Anxiety Attacks which I have not had. They left saying to keep taking my Nexium which I am now on.I was also given a pres. for a Ventalin inhaler.

    They said come right back if I deveop pains or more breathing problems.

    Who else out there has beathing problems? It feels like someone is pressing my chest.

    Note to mjcm123,

    Just returned from seeing my consultant surgeon about the results of my 24 hr PH monitoring and manometry tests. She didn't go into any great detail about them other than to say they showed significant incidences of reflux and the motility of the esophaegus was slightly impaired as the result of the hiatus hernia. She said these findings coupled with the presence of the hiatus hernia and the failure of PPI's meant surgery for the correction of the hernia and my loose lower esophaegal sphincter was the only option. It is a litte frustrating because I told my own doctor this over a year ago and have re-iterated my view in various posts. She stated that in her view this would correct the reflux problem but could not guarantee correction of all the symptoms I have experienced. I accepted this fact and I know that clinicians have to tell you this along with the risks and side effects of surgery to make sure the patient understands. I explained that the symptoms are known symptoms of LPR and occurred around the same time as the reflux. LPR is not a well known illness in the UK even among GI consultants and normally her patients would require surgery to alleviate heartburn. I also told her and she agreed with me that whilst the correction of the reflux may not initially resolve all the symptoms without this taking place I would stand little or no chance of correcting them.

    I have been placed on the waiting list for surgery on the understanding that as soon as I lose about 20lb the nissen fundoplication will take place. Should take me no more than 8 weeks to do this. I did not want to undergo any surgery slightly overweight in any event. It's taken a full year of persistance, private consulations and anguish to get this far so another 8 weeks is nothing. She has personally carried out many fundos which apparently is an important factor for their success or otherwise. So fingers crossed. I will let you know how I get on.

    Hi Ginger. Sounds like your perseverence has paid off! Have your read Blasterboy's thread? He is also in the UK and had a partial wrap. I'm surprised they are asking you to lose weight now as it seems many lose AFTER the surgery. Good luck ... I am keeping my fingers crossed for you!! Keep posting ... I may not be far behind you.     I too have had the breathing problems. My ENT directed me to a specialist who did a videoscoboscomy and a throast xray was done. It was found that they think my breathing problems are because of scar tissue under my vocal cords (caused from GERD) This is called tracheal stenosis and you have to have surgery to correct it!
