Could my strange symptoms be related to my acid reflux or HH?

I am a 24 year old college student from California who was diagnosed with Acid Reflux disease and a medium sized Hiatal Hernia back in January of this year. I originally sought medical advice when I started waking up everyone morning with burning in my stomach. It was a raw feeling that was similar to the feeling of starvation. At first my doctor thought it was possibly an ulcer or bacterial infection. I was put on Prilosec 40mg which actually helped a lot. The burning subsided and I thought everything was going to be alright.

A few weeks later I started to get very nauseous in the morning and before bed. Some days this was relieved by drinking a glass of water and eating oatmeal as soon as I got up. Soon after that the nausea started occurring off and on throughout the day. Then the worst part of it occurred a few days later. I woke up one morning and had some cereal and decided to work out. As soon as I finished I became extremely weak and nauseated. I went to the bathroom and threw up more than I could ever remember. Then I started getting dizzy and felt like I had to urgently use the restroom. I had diarrhea for about 10 minutes then I started getting extreme cramps in my stomach. I honestly felt like I was going to pass out. This lasted off and on for about 90 minutes. The cramps started off at a pain level of 4/10 and progressed to straight 8/10 then it would subside and come back to 8/10 then slowly get better. After that morning, I went back to my general doctor and he sent me in for various blood tests and to a gastro specialist. The gastro doctor ordered me to get a full colonoscopy and endoscopy to check out what was going on inside. The funny thing is I didn’t get the cramping and nausea problem for several weeks and opted out of getting the test done right away.

I started eating really healthy and stayed away from foods that agitated my GERD. I was ok for about a month then once again it happened but this time worse than ever. This time my arms went numb, my legs went weak, I felt like the room was spinning, cramps were 9/10, diarrhea smelled potent, skin went pale, shivers and sweaty, and body aches. I went back and had the following tests full blood lab work including a check for inflammation in the gastro system, endoscopy, colonoscopy, ct-scan, and an ekg. The test came back POSITIVE for Hiatus Hernia, Z-line irregular, GERD, flattened mucosa, and irritation in my esophagus. The flattened mucosa was NEGATIVE for Celiac Disease and my gallbladder was healthy. I went on for awhile on prilosec but decided to take it less often and went down to one 40mg every other day. I felt fine for a while and the only problem I really had was pressure and pain in my hiatal hernia area. Then once again I experienced the same symptoms. It was pure agony. I went back once again and he decided I needed to switch to Nexium 40mg twice a day. I started doing yoga and taking the nexium which actually helped relive some of the pain I was getting in the hiatal hernia area. Then once again last night I got it again as worse as before. So I assuming it has nothing to do with acid since I am taking such a high dose of nexium. I am now considering getting another opinion or maybe even at the mayo clinic in Arizona. My doctor didn’t seem too concerned with the fact I spent 2 nights at the hospital and several nights very sick. ER couldn’t figure it out and the paramedics told me if the doctors didn’t understand it they wouldn’t either. I am hoping someone who reads my story can relate or has had similar problems that have been figured out. I know this was a long post but it was really only a shorten story of what’s been going on.

So once again my symptoms are...

Daily pressure and aches in the chest near the hiatal hernia that seem to hurt more when inhaling but not much while exhaling.

About once or twice a month I will be woken up feeling like I am going to pass out with extreme nausea, weakness, numbness in the arms and hands, followed by subtle cramps then extreme cramps that come and go through a period of 45 to 90 minutes usually. During this time I become cold, shaky, sweaty, and my blood pressure lowers. Afterwards, I start to feel better and am able to go back to sleep somewhat normally.

I take 40mg twice a day currently.
I take fish oil, mulit-vitamins, and calcium.
I eat a well balanced diet.
I do not smoke.
I rarely ever drink.
I was negative for gallstones and gallbladder disease.
I was negative for celiac and crohns disease.
Before this I was very healthy and had no other health issues.
My dad also has gerd and hh but does not have these symptoms.

Hi Jason:

Your symptoms could certainly be due to GERD, though the periodic diarrhea and vomiting sound more like gallbladder to me. Or you can be allergic to something you eat and have a bad reaction to it every so often. It may be helpful to keep a food diary to see if it helps pinpoint a pattern.

In the meantime, try to eliminate coffee, alcohol and dairy products. They all aggravate GERD. If you like milk, try almond milk. I find it very soothing to the stomach and to not cause reflux.

I think it is a good idea to get a second opinion, though it is reassuring that several troublesome diagnoses have already been ruled out.

There are lots of suggestions for remedies throughout these threads, including DGL, aloe vera juice, probiotics, etc. You may want to try one or more and see if they help along with the PPI use.

My other suggestion is to talk to your doctor about switching you to another PPI. They all don't work the same for everyone and some PPI's cause side effects for some people but not others.

I hope you start feeling better soon and let us know how you are doing.

Hi Jason!

You have described all the symptoms that I seem to get as well! I am 26 and for the past 2 years i have suffered every other day. I started to get really severe burning in my stomach about 2 and half years ago, and i ignored it. Then I thought that I had an allergy from alcohol because it only seemed to happen when I drank- so i stopped drinking but it didnt stop. I had an ultrasound, x-ray and cat-scan and was told I was OK and there was nothing wrong with me?! I kept arguing with the doctors telling them I was in agony-and the finally I went for a gastroscopy and they told me that i had peptic ulcers. I went onto Omperazole (like nexium) at 20mg and after 1 month it seemed like I would be alright. But then the burning came back- and with a vengence. I then got moved up to 40 mg a day and it was alright again. Then it came back and they gave me 40mg twice a day and started me on 150mg of ranitidine. They kept putting me on more medication and the problem would subside- then come back even worse. Then in March i went for another gastroscopy and they discovered that I had a hiatus hernia. On 3 seperate occasions i have what i call 'attacks' I get intense stomach craps and throughout my back and have severe nausea and feel lightheaded and dizzy. I went to A&E the first time it happened- I was terrified and they gave me some paracetemol and gaviscon and sent me home after not knowing what was wrong with me! It happened again about 5 weeks later and then again another month ago. Inbetween that I have had 7 incidents where I vomitt for 3 or 4 days straight and feel totally helpless, dizzee and weak. I have kept on at my doctors for answers- my life is being destroyed by this hernia. In the last month I had a barium meal xray and an ultra sound and on the 1st of September I am going to a 24hr manomatry test.

I have been to so many different doctors and been told to wear loose fitting clothing, have no caffene whatsoever, not to bend forward, not to physically exert myself, not to eat anything acidic, not to have dairy replace with goats milk or oatmilk, no oily food and nothing high in sugar. sleep sitting almost upright, dont lay down 2hrs after eating and most definately no alcohol. Also i find that peppermint (chewing gum) and garlic really upset my hernia- i have changed toothpaste and mouth wash to avoid this. It also helps to eat smaller bites and only drink at the end of a meal.

I do my best to follow these guidelines and i find that taking a pro-biotic daily along with milk thistle really helps. There are loads to choose from but I use a generic one from my chemist and it has a high quanity of ginger- which is really great.

I am really sorry to hear that you have sufferend and I hope that you find some comfort in the advice I have given you.
