Red face Reflux & swallowing problems (possible motility issue?) consuming my life! HELP

Trying to make a long story short without leaving out important details, so thanks if you read this and for any advice. or like experiences.

I've had a bad flare up of acid reflux/GERD these past 3 months. before that I had reflux off and on but was controlled very easily. right before the flare up, I swallowed a pill that seemed to have felt stuck in my throat below my adams apple. It could have been just a sensation or it scratched on the way down but it really did feel like it was stuck there. of course it freaked me out as I was thinking it could move into my wind pipe and compromise my breathing. After about an hour, I continued to have this sensation, didn't even eat lunch. later that night the feeling seemed to let up some. I ate dinner fine. after this pill incident my reflux blew up BAD. Never had reflux that badly. GERD friendly foods weren't even friendly. Over the week I realized my food was moving down slowly after swallowing. its very uncomfortable and I can feel it moving down my esophagus. It alarmed me because I never felt my food moving down so slowly like that and never had an issue swallowing. I chalked this sensation up to the pill incident and reflux. I went on a soft diet as I was scared to eat anything really hard incase it made problems worse or got stuck.

I've been to a GI doc who told me after endoscopy that I had no strictures or narrowing but some esophagitis (at the bottom) and gastritis. told me to stay on the soft diet awhile longer so my esophagitis could let up. after endoscopy I guess from being inside my stomach and messing with my LES, it seemed to blow up my reflux even MORE. Doc changed my PPI to Dexilant. Which I am currently on. I told him my symptoms got worse after endoscopy and I developed a new symptom of the dreaded "lump" feeling in my throat. He told me I may want to consult with a ENT if it didn't let up. which it didn't.

I had to go on a completely blender liquid diet (blended oatmeal or cereal, yogurt smoothies, ensures, blended meats and potatoes into creamed soups) because I am still having problems swallowing and because of this new lump feeling in my throat and the reflux that was bad for a week after the endoscopy, it made the issues swallowing that much more troublesome. Tried to eat the soft food again and it felt the same- going down slow, but I just didnt want to deal with it anymore.

Went to ENT had laryngoscopy which he said was normal. ENT thinks that I could be having a "motility" issue with my esophagus. I did a barium swallow to check for my problems swallowing but nothing was said about the motility or swallowing but my reflux was "moderate to heavy" Which is crazy cause I've been on PPIs over the course of these 3 months, which some didnt work, but I've been on dexilant for a month and a half now.
So I've been so depressed and worried about what is wrong with me because none of the tests are stating anything about my swallowing problems which I still have and also, I'm not having problems really with acid in my throat, but I'm still having reflux (as the swallow test confirmed) of food from time to time but with no acid from the stomach. sometimes it feels almost like the food refluxes within 2-3 minutes of swallowing, which makes me wonder if it really could be a motility problem because the food is taking longer to go down.

I've been on the blender liquid diet for a month now and I'm really depressed and scared. I'm worried if I do have a motility problem that I will have to eat the rest of my life like this! I have an appt with my GI doc and I know the next test he might do is a manometry test for the motility which I've heard is really uncomfortable. I just dont know what to do anymore. this is really putting so much stress on my quality of life. I never knew acid reflux could be so bad! If anyone is going through this or has gone through this please share your story. I need all the help I can get. I feel so helpless. And now I feel I'm going to develop anxiety towards eating. Also, to anyone who has been on a liquid diet for months at a time or is currently doing this, how do you mentally cope with it? I think out of everything this is what is causing me the most grief. I can't even enjoy thanksgiving with my family this year =o(

wow, I was hoping to hear back from someone. does anyone have an esophageal motility disorder or a situation similar to this that you can't eat?

I have searched for other posts about esophageal motility problems or other problems relating to reflux and not being able to eat but they are all outdated.

Re: Reflux & swallowing problems (possible motility issue?) consuming my life! HELP

I know how you feel. I posted under LPR/Acid Reflux - Who Knows back in September or so and see over 1,000 people read my story but only a few replied. Not sure what that tells me, I have my ideas.

Anyway, I do know how you feel. As I write this, it feels like I have something stuck in my throat, I get what I call volcano type pressures in my sternum area whre your LES is from what I'm told. Sometimes I belch and other times it will subside. Some days are better than others. My diet is pretty good so really doesnt matter what I eat or drink. Been on Nexium for 3 mos now with no real change. Maybe less belching but has done nothing for my throat. Had a barium swallow, endoscope, flouroscopy, ultra sound and blood work. All they can come up with is a possible motility issue with my esophagus. I share the findings with other doctors I knwo fromt he health club I belong to and they just shake their heads. One retired internist told me gas goes one of two ways and I am just gassy. A retired ENT told me their could be 100 reasons why I have phlegm build up in my throat. All the blood work came back normal. No allergies.

All the posts you read, they all start to sound the same. I think a lot of people, including myself, came on here thinking they would find a cure because someone else had a better doctor or better testing and the realit is, we have all had the same tests and the same diagnosis. I just don;t think the doctors really have a way of knowing beyond a reasonable doubt.

I heard a new term just yesterday, Cricopharyngeal Muscle Spasm? This is just another on a long list of possibilities that seem to have some of the same symptoms of all the other possibilities.

I wish I had some better information for you or a tip on how to get rid if it. I keep thinking I will click on someones post and see something promising.

    Mark

Re: Reflux & swallowing problems (possible motility issue?) consuming my life! HELP

Hi mark. You are right! I do come on to these boards thinking I may find someone who has actually came out of their health trial and maybe had some suggestions or something. even a good ending! but as you said its all the same. I did read about the cricopharyngeal spasm. It could be a possibility but as I have mucus or a lump in my throat, my throat doesn't always have that "tight" feeling that accompanies the spasm. The mucus and lump feeling along with the slow moving food in my esophagus are my main complaints. I could even maybe live with the mucus and lump, but the motility thing is what has me worried! My GI doc said it could also be spasms in my esophagus.

are you having problems swallowing too? you said they told you it could be a motility issue with your esophagus. do you feel your food moving down slowly or feel it not moving down right? does what your drinking or eating come back up into your throat as your eating, not after like acid reflux. this is pretty much food or drink coming back up into your throat without mixing with your stomach acid. the reason I'm asking all of this is because this is whats going on with me and I actually thought it could be a motility problem which was also said by my GI doc once I seen him about it. but he also said it could be spasms. he is sending me for the manometry test for the motility in December. did you ever have this test done since they are saying it could be a motility problem for you?

Re: Reflux & swallowing problems (possible motility issue?) consuming my life! HELP

Yes, I do have the mucus and I do have the lump feeling in my throat. I do have trouble swallowing but I do not seem to have any trouble eating and swallowing. I do belch off and on while eating but the interns I know asked if I eat fast and said I may be swallowing air. They have also asked if I belch voluntarily or involuntarily. If involutarily, do I try to force more out than my bosy wants to release? That can cause food to come up. I do feel food is stuck somewhere in the espohagus and may be agitating up and down like a washing machine agitates. They said that can be caused by faulty muscles or nerves. I read about the Vagus nerve and what it does. That could be it? The ENT suggested Reglan for the motility issues but I declined at this time feeling the side effects out weighed the potential benefits. The ENT didn't push me after that so I would think she didn't think that much of it either. Where I live, they only do the manometry test by placing the wire probe in your espophagus for 24-48 hours. Even she and surgeon I know said it is not really that conclusive. There is a new one out there that puts the probe in by endoscope. Your body just gets rid of it after a few days and it is non invasive. May ask if they have that and see if your insurance would cover that procedure. I will turn 50 next month. I think this is just what I have to look forward to for next 25 or 30 years or until they do discover a cause and cure.

hi Mark, since you do not have any issues swallowing food I think you are probably just suffering the many ailments of acid reflux. maybe the food that feels stuck is the acid reflux mixed with food that is coming back up into your esophagus. I belch sometimes too but belching is also symptom of acid reflux.

actually the manometry testing they are doing for me is they stick a scope up the nose and have you swallow to push the scope down the esophagus, they then have you take sips of water as they pull out the scope to measure the contractions of your muscles as you are swallowing. its only about a 10-15 min procedure. you dont have to keep anything in you for a one or two day period. and nothing is released from your body. its much like the scope that the ENT does when they look down your throat through your nose except they go into your esophagus and test your muscles as you swallow. and from what I know this specific manometry test is very accurate in diagnosing motility problems.

Wow, I haven't heard of that test. Sounds way better than the other options and especially if it is that accurate. Can't wait to hear back from you once you have had the test done. My whole problem really is the fact that one day in 2010, I was fine and the next I wasn't fine and haven't really been fine since. I lift weights 6x a week, do cardio of somekind 6x a week, eat a pretty healthy diet with a few exceptions, sleep good, not really stressed, don't fit the so called model of someone with reflux, don't have heartburn, don't wake up at night. Just have frequesnt belching, felling of something in my throat and scratchy throat. Did have the eNT tell me the tips of my vocal chords were red so she says it is LPR. I am so sorry you are so young and have problems. I really didn't suffer from anything until 49. I have read of kids in their teens with these issues. I just can't imagine and feel so bad for them. Please keep me informed on your test results. Good Luck!
