    Lower Back Pain from Auto Accident

    Good morning All,

    About -6- weeks ago my car was hit in an auto accident where my lower back on the left side was injured. I have been to the ER and my family doctor, to date I have been prescribed Oxycodone 7.5mg, Gabapentin 300mg, and Cyclobenzapine 10mg to be taken daily to help with the pain, which to be honest, just is not cutting it. I do have an appointment with a Back Specialist this coming Monday and was hoping to get some advice on what I should ask for in a stronger pain reliever, prior to seeing the Specialist, so I do not look completely stupid. Can anyone advise me.

    Welcome to the board. I am sorry you are suffering a back injury from a car accident. Unfortunately this can result in an injury where it can be difficult to find the extent of the damage at this point. However seeing a spine specialist is a first step.

    You do not mention your age, but if you are relatively young, I would suggest you not ask for any medications by name. Instead, be prepared to accurately describe the results of the accident in detail...specifically where you have pain, when you have pain...what activities make it better or worse, whether it is better or worse at a particular it,e of day, etc. rightly or wrongly, some doctors worry that younger patients are "drug seekers" so you need to be careful how you respond to the doctor's questions.

    I assume you are going to the spine specialist because you want an accurate diagnosis and plan for treatment so that you will recover to the best of your ability...so that you will heal from your injuries and recover as much of your former health and vitality as possible, and that you are not going just to get stronger drugs. In reality what you have been given is about as strong as you are likely to get.

    At your appointment a typical appointment would consist of a basic neurological exam, physical exam and review of any imaging you may have had. Treatment would initially begin with a course of physical therapy to strengthen core and back muscles and various other modalities that would be appropriate for your injuries, oral medications for pain and inflammation and perhaps a series of epidural steroid injections for the same reasons. Only when conservative treatments have been tried and failed would surgery be considered. Without knowing more specifics about your injuries it is impossible to say anything more specific.

    Good that you did not get serious injury. Talk to your doctor carefully about your medication. Thorough medical evaluations should be conducted to give you the right treatment. Sometimes you will be needing therapy like chiropractor which deals with your bone. Also you can do some remedies to support your medications. Just take it as prescribed since it can be addicting.

    I absolutely would not recommend any of the above exercises till you have seen a spine specialist. You could really do a lot of further damage. Since you don't know the extent of your internal injury it would be wise to ice it, take your prescribed medication and possibly some anti-inflammatories like Motrin. Gentle stretching and walking would be the most I could see you trying. No twisting or bending if possible.
