    Dear Posters

    Please advise with respect to the following. Please mail me if you need more information. Any help is appreciated. Thank you.

    Problem: No menstruation from Aug 2009 to Jan 2010. No sexual interactions of any kind from April 2009 - Nov 2009. Protected (condom) sex a couple fo times from Nov 2009 to Jan 2010. Started spotting continuously from Jan 2010 to Mar 2010. In Mar 2010, blood work came out normal except FSH and LH levels were reversed. Physical exam was normal. Was prescribed generic form of Provera (10 mg/day for 10 days) to induce period and Loestrin 24 Fe to make periods regular. Spotting stopped as soon as I took Provera. 2 days after stopping Provera, period started (Mar 6). Doc recommended starting Loestrin 24 Fe on Mar 7. First pack of Loestrin gets over tomorrow but bleeding has not stopped since Mar 6. Transvaginal ultrasound 2 days back came out 'normal' -endometrium was as thick as it is supposed to be for this stage of the cycle. No anemia. Bleeding pattern- changed Kotex overnight pads every 4-5 hours during the first week, every 2-3 hours in the second week and every 6-12 hours during week 3. Today is week 4. So far same pattern as week 3. No cramping at all during this entire time. Some lower back pain. Large blood clots during Week 1 and especially Week 2.

    Age : 30 years old

    Weight/Height: 180 pounds, 5 feet 0 inches

    Sex: Female

    Start of menstruation: 12 yrs. Regular 28 day cycles with predominantly moderate levels of bleeding for 3-5 days each time and some occurrences of temporary heavy cramping. Menstruation stooped for 3 months at 18 (cyst diagnosis. resolved with medication), 3 months at 24 (low thyroid level diagnosis. self-resolved) and for 6 months from Aug 2009 to Jan 2010.

    Medical History: 1. Diagnosis of small ovarian cyst (s) in left ovary at 18.
    2. Low thyroid levels at 24. 3. Insignificant (size) microadenoma of the brain at 24. Cysts resolved with medication. Never treated for low thyroid levels. Treatment was not recommended for microadenoma.

    Birth control: On Yaz from Nov 2007 to Dec 2008. No bleeding during withdrawal pills. One occurrence of bleeding on active pills in Nov 2008. After stopping Yaz, natural menstruation took place in Jan, Feb, April and August of 2009.

    Hello Maravan,

    Hmmm ... sounds to me that if the ultrasound came back normal then other causes for the irregularity should be investigated such as thyroid and the microadenoma. Have they kept an eye on these other issues since when you were 24 years old ? You seem to have had other incidences of this in the past so perhaps a more in depth investigation might be in order. Playing around with hormones to try to get you to fit into the 28 day cycle is all well and good but if there is an underlying problem that has to be fixed, it should be addressed (ie. perhaps a thyroid issue).

    If your doctor does not seek any further, see about getting a referral to a specialist such as an endocrinologist for example.

    Hi Estria

    Thank you for the response. My thyroid levels came back normal. So did the prolactin levels which apparently is reflective of the microadenoma not being a problem (even when it was first discovered it was ruled out as the cause of the problem).

    Hi Maravan,

    I noticed that you did not menstruate during certain months after going off YAZ. Has your doctor performed any other tests such as an endometrial biopsy and a pap smear or HPV test ? Also, are you seeing a general practitioner or a gynecologist ? A gynecologist would be highly recommended under the circumstances.

    Obviously the hormone pills that you were prescribed are not managing to control the bleeding so you should discuss this with your doctor to see what the next step should be. Irregular bleeding can be due to a number of things (including fibroids and even the pills that you were prescribed) and all of them should be ruled out before a diagnosis of DUB is given (dysfunctional uterine bleeding). If you continue to bleed and spot, a procedure called a D & C can often take care of the problem so you may want to discuss this with your doctor.

    Let us know what your doctor says. Obviously further investigation is necessary and playing around with hormones is not the solution. You may have to demand further tests to get to the root of the problem.
