    Need help with painful fissure

I am going to be as short as possible, but it still will be long. Since February 2019 (We are now in August 2019), I have suffered from an anal fissure without knowing it.

The pain got so bad during and after bowel movements that I decided to see a gastroenterologist.

I thought I had only hemorrhoids and that they would go away but did not. I tried all the hemorrhoids creams you can imagine, online and in stores and got the same results, they all burned me more. No help at all!!!

When I saw the doc, I was surprised that he didn't inspect me down there as I had read online. He basically took my word for it and prescribed me suppositories for the hemorrhoids. They did not help. When I saw him 3 weeks later, I told him I was still in excruciating pain and couldn't sleep at night.

He told me then without again inspecting me that I didn't have hemorrhoids but a fissure.

He prescribed me a cream which helped a lot for 2 weeks and then it got worse again. (The cream was Nitro-bid.)

Upon seeing him again I told him that the cream helped but then stopped working and he replied "It will take 8 to 12 weeks to heal completely, you don't need to see me again".

I was completely choked by his reply. The guy never inspected me !!!
Anyway I decided to go see another specialist. This guy within 5 minutes of being with me, had me butt naked and was inspecting me.

He told me after 1 minute of looking that I had 3 hemorrhoids and a small fissure.

He recommended banding the hemorrhoids and gave me a cream for the fissure.
Upon seeing him for the first banding session, prior to band me he looked in there and told me that the fissure was healing nicely.

He banded my first of 3 hemorrhoids and told me to keep using the cream for the fissure. (If you need banding for hemorrhoids and are scared, don't be, it is quick and painless).

At any rates it seems the cream stopped working just like the first doc.
I need to tell you now that for the last 10 years I have been eating like a pig and am paying the price.

I am 52 Y.O. male and was about 210 lbs (Down to 190 right now). I was stuffing my face at every dinner meal. I would eat 2 ribeye steaks and fries with half a French baguette. I love sushi and would eat between 50 and 80 at the buffet or even fast food at BK or MCD with 2 large double whoppers and large fries.

Well you get the picture. In brief I was killing myself.

In the last 2 months I have changed my diet by 100%.

I eat fruits, veggies and beans mostly along with some fish sometimes. I allow myself a slice of sprouted bread per meal. I use olive oil to cook it all, or on the salads as dressing. I hardly eat any meat, maybe a little piece once a month.
I also drink 4-5 quarts of water daily and take powdered fiber supplements.
I have dropped weight but still the fissure causes a lot of pain.

I wished someone would explain me why is the fissure painful when you have a BM, then it stops hurting for 2 hours and then hell starts for the next 3 hours of on and off of excruciating pain. Only 7 hours after the BM, am I completely relieved of all pain.

The burning is awful, I don't know what to do with myself. Last time I had an afternoon BM and after that while driving, for the first time I experienced spasms. If you don't know what spasms are, for me it was as if you take an extension cord, put it in the outlet and stick the other end in your butt.
It lasted 15-20mn while driving. After it stopped, the right side of my butt was completely numb, absolutely no feelings for 5 minutes down there. It was scary.
    Then the feeling came back.

I called doc to prescribe me pain meds. He called in "Tylenol 3" which is no help.
I have been reading this board and others and am trying to find some relief.

I am on my way to CVS to pick up something called "Nupercainal Hemorrhoidal Ointment" which I read a lot of good reviews from fissure sufferers.

If that doesn't help in a week, I will order Doctor Butler's Hemorrhoid & Fissure Ointment" which gets fantastic reviews for fissures too.

I have no faith these products will heal my fissure but I hope to get some relief.
I have no doubts I will need surgery but I will see what the doc says in 3 weeks when I see him.

Can't take that pain any longer.

If you have any tips to help with the pain I would appreciate it.
