Hit my head and consistent symptoms for 3 days

I was playing with my children in the playground.
I hit my head hard on the top (roof?) of playground slide. The top is made of plastic but it was hard.
I did not black out or see stars etc at the time of hit.
However, I started to having headaches later that night with nausea.
It has been three days, and I am still having headaches, dull pain on the head, sinus problem (nose, and throat pain), nausea and tingling on the face.
The tingling on the face comes and goes.
My right arm and leg feel somewhat weird. I feel like I am having sensory issue on the right arm and leg. (I had herniated disc from the car accident about 6 months ago, so maybe these right side pains are related to the spine problem)
Does it all related to the hit on the other day?

I am so scared, because I still have symptoms. This nausea and dull headache bother me a lot.
Fortunately, I have a doctor's appointment tomorrow, so I am going to mention it, but I am not sure if I should go to ER now to see the doctor or wait until tomorrow to see my PCP.

Sounds like you have a concussion. It's a little late tonight to go to ER if you haven't already been earlier today. Glad you have an appt tomorrow. You do need to see a doctor.
