What is the difference between enlarged ovary and complex cyst

A friend of mine (40 yrs. old, no children, recently diabetic, infertility issues) is having surgery next week to remove what was called a complex cyst-7cm. The gyn said that it is actually the ovary that is enlarged so it is tricky. They will probably have to remove the ovary and hopefully not have to do a complete hysterectomy. Which would be good since this was all found while doing a fertility workup.
Is a complex cyst and enlarged ovary one in the same, or two separate issues?
I always thought that a cyst "grew" on the ovary, but it seems like it is actually inside enlarging the size of the ovary?
So what are the chances that the entire ovary will have to be removed?
So there is a cause for concern of cancer, of course, but there are no other symptoms so hopefully that is a good sign.
The radiologist said it just looked like a cyst and to recheck in 6months, but gyn wants it out probably because of the fertility issue. However, the gyn said she really doesn't think it is cancer. But maybe that's just what they tell everyone so they don't panic.
We are really hoping it is just a cyst.
Any help appreciated....

To my understanding, a complex cyst grows in the ovary and thus enlarges it. Under the right circumstances, a good surgeon can remove it while keeping at least a piece of the ovary. Mine was 12 cm and they were able to salvage part of mine.
