    Thanks for all the replies but I think if I can go for surgery itíll be my best bet my bone scan came back negative and my CT scan hopefully will come out negative I just did it yesterday I am meeting the urologist on Monday and hopefully I can go ahead with the surgery I have to take the chance and pray that it isnít spread the only way Iíll really find out is after the pathology report and they give me my margins or I have my lymph nodes are cancerous I really donít wanna have radiation and hormone treatment The urologist told me that itís a 7% chance of spreading Because my scans were all clean life is never fair And getting cancer is one of them and losing your manhood is another but there is really no other choice but going forward or youíll end up going backwards and you wonít exist hopefully I can get through this surgery in there with it alone enable to take care of myself is going to be the hardest part and dealing with it alone but thank you very much for all your help and consideration

    Phil,

    I read your post about taking care of yourself. Let me assure you that it will not exactly be EASY but certainly doable. Except for letting my wife take care of me so that she felt like she was doing her part, I could have done it alone if i had to and i wasn't in shape.

    Driving and getting groceries will be the biggest issues but maybe opt for food delivery and grocery delivery services. Once the catheter is out, life gets much easier.

    The biggest surprise of the surgery was that it was a lot easier than I anticipated from a pain and immobilization perspective. You might want to have a friend, coworker or family member check in on your from time to time just to make sure some unanticipated issue didn't arise.

    I can honestly say that my should slap tear surgery was much worse from a difficulty perspective.

    Sorry, and welcome to the club. I am 60, just diagnosed, and doctor wants to cut it out. I'm going for a second opinion. I'm not ready to give up sex yet, so I hope radiation is possible instead.

    Thanks. I've tried up to 20 mg of cialis and nothing.

    The doc said the shots were next. I'm not sure if I'm ready for those, they don't sound entertaining.

ADT Lupron as first therapy, in Dec 1999, then + Casodex in March 2000, then + Proscar and Fosamax in Sep 2000. Rejected for surgery January 2000; offered radiation but told success odds were low; switched to ADT only vice radiation in May 2000, betting on holding the fort for improved technology; PSA gradual decline to <0.01 May 2002. Commenced intermittent ADT3 (IADT3) with first vacation from Lupron & Casodex. Negative advanced scans in 2011 (NaF18 PET/CT for bone) and 2012 (Feraheme USPIO for nodes and soft tissue). With improved technology, tried TomoTherapy RT, 39 sessions, in early 2013, plus ADT 3 in support for 18 months (fourth round of IADT3), ended April 2014. Continuing with Avodart as anti-recurrence shield. Current PSA remarkably low and stable at <0.01; apparently cured (Current PSA as of 9/4/2020). (Current T 128 9/4/20.) Supportive diet/nutrition, exercise, supportive medications during this journey, as well as switches in antiandrogen, 5-ARI, and bone drugs. Barely noticeable side effects from radiation; continuing low T, likely do to long use of ADT, but good energy and adequate strength. I have a lot of School of Hard Knocks knowledge, and have followed research, which has made me an empowered and savvy patient, but I have had no enrolled medical education.

    I had mine taken out Nov 2019 and so far good psa tests, I have another one in a week and half.
    I echo some of the others on here about the surgeon if that's the path you choose, make sure they have a lot of removals behind them.
    I went to MDAnderson in Houston for my surgery and I am from OK, but it was worth it to me to find the best I could.

    One night in the hospital and out the next day, but it wasn't as easy for me as it was for ICE.
    I struggled quite a bit for the first couple days, trying to walk as much as I could to get rid of the gas pains.

    Continence good soon as the cath came out, got a pump right away and used it everyday for the first 6 months. Erections good with shots but somewhat painful for me, not so much with pills. Taking sildenafil daily still.

    I have natural erections coming back now about 50% so I have a lot of hope they will get better with more time.

    Good luck on the path you choose, lots of good advice on here so keep asking questions and you will get answers.

    The emotional part was the hardest. You can check in here. Physically, I managed very well. It is major surgery so care for yourself as much possible. Take time off if you can.

    So I met the urologist today finally for my final decision for surgery and I got the bad news that he has to remove all my nerves due to all the cancer in my prostate the 12 positive cores out of 14 so basically Iíll be castrated no possible use of my prostate the good news is he thinks if he does that I have a 7% chance of spread also my MRI
    CT scan and bone scan showed up clean heís done over 2000 robotic surgeries so heís very good he was straight with me and told me that I need to have this out since itís still in my prostate Itís crazy to believe at 56 years old and so healthy and everything working down there Iíll end up like this but I have no choice see the cancer to be cured hopefully or death if it spreads anybody reading this be grateful if you donít have cancer itís not a pleasure to have and lose your manhood my life is going to change into weeks and I canít do anything about itI want to thank everybody that replied to me and this board

    Phil,

    I'm puzzled that you have such a fear of IMRT plus ADT (hormone treatment). Make sure you check with a radiation oncologist that knows PCa. I am on withdraw from 6 months on hormone treatment that I had along with 28 sessions IMRT. Other than being on a few months of pseudo menopause (women do it all the time), it has been a big nothing burger.

    I have been fully sexually functional the entire time. At least 10% of men are and I think more could if they tried. Just a different kind of sex that requires foreplay that my wife and I are enjoying. No bowel problems, no urinary problems other than a bit slow as it was before treatment.

    Diagnosed at age 73 Feb 2019 DRE indicates nodule PSA 2.8 Aug 2019 PSA 3.1 Urologist suggests biopsy in Oct Results of biopsy: 2 of 12 cores positive. Low volume T2b, intermediate risk, GS 3+4, PSA 3.10, prostate cancer, perineural invasion. Followed up with MRI to help decide between surgery and IMRT. MRI shows suspicious PIRADS 5 lesion measuring 2.cm in diameter, with associated left neurovascular bundle involvement. Started 6 month lupron series Feb 2020, 28 sessions of high dose IMRT Apr 15, 2020. Sexual functions okay except ejaculate has changed. Without libido it is an academic process that requires much focus. July 27 first measure of PSA and total testosterone. PSA: .13 ng/dl Total testosterone is less than 12 ng/dl.

    Well I finally met through urologist today and theyíre going to take all my nerves out of my prostate they have no choice due to all the cancer thatís in there I didnít expect to hear that today but thereís no choice itís not worth the chance to leave them in for the sake of cancer spreading the only good outcome is the bone scan and a CT scan and the MRI scan showed no spread the bad news is my prostate will no longer be effective at 56 years old thatís a terrible thing to have especially if you were fully functional and very healthy the oncologist agreed with this assessment and due to my age he thinks that surgery is probably my best option so Iím hoping for the best in the pathology report shows no margins cleared or lymph nodes issues Iím due for surgery in two weeks and hoping I turn out at the end to be cancer free thanks for everybodyís help And advice we all get a set of cards dealt in our life and cancer is never a good choice and neither is losing your manhood kindest regards Philip

    Hi Phil, Feb. 2018 I was diagnosed with prostate cancer. Urologist told me either removal or radiation. I weighed the options after reading up on the differences. I chose 5 weeks of radiation 5 weekdays for 5 weeks. I started Dec 2018 and finished the first week of Jan. 2019. I never got sick. I lost stomach and chest hair but nothing else. Psa since then has been 0.1. With either choice, you will lose your libido. I currently am using Cialis for that. You will have dry ejaculations. If I had a different insurance, I would have chosen Proton radiation therapy. Read up on that. It is less evasive than external beam radiation. Good Luck.

    Phil, I'm very sorry brother. Prayers your way.

    I know it is hard to face such an Everest at your age. But sometimes life just sucks, there is no way to put it other than that. But we all play the hands we are dealt the best we can and hope for a good card on the river.

    I can tell you now that I'm on this side of the surgery, I'd still do it again even though it left me impotent so far. Life is about so much more than sex and I find life is as good now as it was before. Just different.

    But having been there, I also know that there isn't much that I can say to make you feel better at this point. You just have to grow into acceptance and you will get there.

    Prayers brother, when is your surgery and where are you having it?

    With your pathology did the URO explain you likely will require salvage radiation at some point after surgery for recurrence?
