Question muscle tone gone is it normal?

My mom has severe and I mean severe rheumatiod arthritis, she is 73 and has been in wheelchair for last 20 so years just this last year, she has fell off as we call it here in a southern state, all her muscle mas is gone, her wrists are like a childs, I thought she'd been that way since her last surgery (she has her spleen out due to enlargement caused by RA) but I got to looking at pictures and she is right, I may have been in denial, I don't want to loose her, its winter now and they never got the flu shots here in time to take her before it got so cold, and she needs her pneumonia updated also, I am afraid to take her out now in this weather with her so fragile and she refuses to go anyway, and refuses to go to a hospital. I seen this happen to my grandpa, the year before he passed, looking back, is it in the normal "package" of things that happen when your body is getting ready to quit? I just wondered if anyone else knew or has experienced this, she has no other symptoms of anything else like any cancers, I have scadds of medical books and have poured over them for symptoms....anyone have any help or advice....
