    I have appt, today. Help please I have all over body pain

    Today I have a doctors appt. for a check up and osteomanipulation. I want to make this appt a good one. I have been having chronic pain now since September. It started out as tmjd, and it was horrible. I thought it was a flair up but it stayed, and I have chronic face, migraines, and jaw pain daily. To add to all that I now have body pain in my hands, with swelling, feet, ankles, knees, sometimes my sciatic nerve, lower arms-muscles, lower legs-muscles, In the morning it is horrible, and I can barely walk. When I get up from sitting, I am sore, and it takes me a while to get going. I have somedays where I have been in bed or on the couch, I am in so much pain. I am 31 and I have 2 small boys, 2 and 4. I stay at home, and try to do the best I can. My husband does not understand why I have this much pain. He feels people have pain as they get older, and I have gained 15 pounds over the last 6 months, so he feels that might lead to body pain. Why can't I deal with it like other people do?

    I am on pain medication, oxycontin 40 mg 2x times daily, vicoprofen for breakthrough pain, flexerill and valium for muscle spasms, and lexapro for anxiety. My husband also feels that my doctor is overmedicating me, and I am not as sharp as I used to be. I have been on the oxycontin for about 2 weeks and I now feel like l have some of my life back. I have good days! I still have bad days, but I am not in bed all day. I have breakthrough pain now once or twice daily, but that's it. I used to have to manage it constantly with the vicoprofen and it was hard. The vicoprofen would only last 3-4 hours, and then I would be in pain, and I was taking too much ibuprofen over the course of the day. The vicoprofen was not covering the pain anymore. So the doctor switched me. At first, I was uneasy about it, and my husband did not want me on it. I am now so greatful for the switch.

    Here is where I need help. I have been diagnosed with tmjd, and myofascial pain syndrome, but that is it. Can the myofascial pain syndrome cause all over body pain? I just saw another oral surgeon last week and he dx with M.P.S. My doctor has said that my body pain is from extra cortisol being released in my body from stress, mostly from my marriage. I think there is something else wrong with me, and I am scared. Last night, I walked around the mall for 1 hour, and got my hair done. While I was getting my hair done, my feet were in so much pain, an 8 on the pain scale, I almost had to stop getting my hair done. I was this close to crying. I was so discouraged. I took the vicoprofen and it got better. Thank goodness.

    I am waiting for insurance approval for the surgery to help my jaw, either athroscopic or arthrocenthesis. Is it time for me to ask my doctor for a referral to a rheumatologist, and neurologist? The last 2 surgeons I have met with both have suggested that since I have body pain, migraines, and it is my left side moslty effected, and my left eye is slower blinking than my right, I should see a rheumy and neurologist. My face is also extremely assymetrical, possibly due to the over use of left side.

    I know I have written a lot. I just need help finding out what is wrong with me. Maybe my husband will be more understanding if I had a diagnosis for my body pain. Like I said, I am scared. I am only 31, and I feel like I am 91. The body pain has changed my quality of life.

    Can someone tell me how fibromyalgia, arthritis, or another autoimmune disease that causes body pain gets diagnosed? I wish my doctor would have started blood work a long time ago. Also, is vicoprofen, I have to take 3-4, the best for breakthrough pain?

    Thank you all for your time. I appreciate all of you reading this, and I am so greatful for any help that you guys can give me. I am close to desperate.

    Here's what I learned:

    MPS is a chronic disorder that affects the connective tissue that covers the muscles. There are trigger points that can cause local or referred pain (pain elsewhere), and there's no reason to think that it must be limited to one location. So I guess the answer is "yes," you can have pain elsewhere on your body.

    MPS can be caused by or aggravated by fatigue, repetitive motion and lack of activity (like an arm in a sling). Stress can make MPS worse. If your husband wants to help he can provide a more supportive environment. One of the most critical areas of support would be for him to accept your need for pain medication. I struggle with this as well in my family. Luckily my wife is very supportive, but my father has never supported me in this. So when we visit him I have to just tune out his prejudice.

    Regarding Vicoprofen - that's not really a good BT med, since the ibuprofen is so hard on your digestive tract. It would be best to get on something like immediate release oxycodone. I use OxyDose for BT pain. It comes as a liquid and the formula is 20mg per milliliter. The dropper goes to 1ml, so that's 20mg and I am allowed 1-2ml every 6-8 hours. Of course, if I need it sooner the doc allows it so long as I don't go over three doses per day. OxyDose is generic for OxyFast, which is the brand name. You can also get immediate release tablets too. All that's in these meds is oxycodone, the same med as in your OxyContin.

    One thing about your OxyContin - many of us have found that dosing twice per day doesn't quite cover our pain and require dosing three times per day. This is not unusual. If your pain increases, you might suggest this to your doctor.

    Welcom to the boards and I feel for you and all of the pain that you are enduring. I am only 33 years old and have been in chronic pain since May 2003. It has been a really rough ride and one that is still difficult sometimes. I would definitely suggest gettin a work-up with a neurologist but also ask you doctor to refer you to a pain management doctor. They are specialist in the pain management field. There are several different kinds though. Some only treat with medication. Some only do procedures like epidural injection. Some do procedures and prescribe meds. They really know what they are doing concerning the pain meds and treating a person long term.

    There are a couple of things that I wanted to say concerning your meds. I would definitely try the 3/day dose of oxycontin which provides a steadier level of the drug in your body and works alot better for most people. I like Steve's idea about the Oxyfast although I would think alot of doctors would be a little hesitant about prescribing this. I would suggest asking for Percocet as BT medication. It might work better than the vicuprofen. I have been having symptoms of TMJ and just got fitted with a mouth guard. I know a little bit about the pain in the jaw but nothing to the extent that you are experiencing. I don't really have anything else to add since I am knowledgable concerning your other diseases. I can tell you that stress will make the "perception" of the pain so much worse. Your pain might be a 4 on the pain scale but as soon as you get under stress it shoots up to an 8. It is really amazing how the mind can trick you into thinking that your pain is higher than what it is. Although I am sure that the cortisol definitely has alot to do with it as well. Too much of that flowing around in your body will cause constant pain and tension. Keep us posted concerning your pain.

    Hi, Kass:I have chronic myofascial pain and have had it for 8 years.* It affects every inch of my body at one time or another.* It is very much an all-over pain disorder and according to a med journal I read in my doc's waiting room last week, the AMA has actually upgraded it to a disease instead of a syndrome. There are some great websites out there that can give you more info.* Just google chronic myofascial pain and you'll find a lot.* Just beware of the sites that lump it in with fibromyalgia.* They may have some symptoms in common but they are very different.* I have been blessed to find a great PM doc, who has an extensive knowledge of CMP.* He has me on the meds and treatments that work best for me and he always listens to me when I feel something isn't working or if I feel that I need more tests, etc. I'm sorry to say that there's no cure for CMP, but it can be managed pretty well if you have good communication w/your doc.** I hope this helps...I'd be more than happy to answer any questions that I can.* Good luck,* cmpgirl

    Kass, I noticed in your post you mentioned osteomanipulation. I have just been referred to see a specialist for rib pain so I was wondering if you have recieved treatment & how well has it helped? this is all new to me & there is a long wait to get in for this treatment so thought I would try to find out alittle more info on it. I am sorry to here you are in so much pain & can certainly indentify with you there. I have alot of chronic pain in the left side from the shoulder area down through the ribs. Just had shoulder surgery but still experianceing alot of rib & chest pain. My doc. just upped my meds, so I am taking 2percocets every 4-6 hrs, still have lingering pain even after taking the new dose. Was on oxy which helped alittle more but always walked around super tired, the oxy was dropping my BP. I would take it just at night & it did help during the night with the pain, but still was waking up. After surgery the surgeon just kept me on perc. & am trying to muddle through.

    At this time he feels the ribs may have shifted to accomadate the shoulder problem so wants me to look into therapy, when I came across your post I thought I would see if you had any comments on this sort of therapy. Sorry to get off track alittle here but since I have never heard of this before thought I would take my chances that you may have some input.     Sammy01, thank you for writing me. I am glad you asked, and I would love to help anyone I can. I go and see my doctor every 2 weeks and he does osteomanipulation on my jaw, upper back, shoulders, sciatic nerve (new one..yesterday), neck, and my head for migraines. I have a great relationship with my doctor, and he is also my prescribing doctor. I have been seeing him off and on for about 1 1/2 years for migraines. Then in September, I started having extreme jaw pain, which then traveled through my upper body. I am now having problems with my feet and hands, muscles and joints.

    Enough about me, do I think it works, yes. For me, he tries to exhaust the muscles so they will not spasms anymore, and works the muscles so that they are not so tender (trigger points). He does a lot more, I am sure, but it is worth a try. What I love about it is that he tells me where I have pain, by touching my points on my body. All I do is say my neck hurts, my jaw still hurts etc... and he touches the points works with them and then the pain goes down on the pain scale. I do have to tell you that some times I am in more the day pain after, and then it does it better. He did warn me that it sometimes happens.

    My doctor kept me on my meds that are working. Today, though, I am extremely fatigued, more so than I have been in awhile. I had a full blood work up yesterday for thyroid and arthritis. I wish I knew what was wrong with me. You said that you were on oxycontin but it made you sleepy? I have been on it for about 1 month and I was sleepy today. It must be something else that made me tired. When you said that you are muddling through it all, do you mean pain? Please write me back if you can, and good luck.     Hope you don't mind me jumping in here, but thought I'd just add a few things:

    (1) Oxycontin can take quite a while for your body to fully adjust. Not surprised at all that you are tired, given your condition and the side effects of the drug...After all, the Oxycodone is in your system 24/7 and it's a potent med.

    (2) When in pain, the body is under extreme stress, which further drags you down, mentally and physcially.

    (3) I think Dr. appts, tests, and etc. also tax you and drag you down...They do me! I get stressed before and during these visits and when they're over, I'm like "whew"....It's over. As with every appt one goes to, they usually take your blood pressure, and mine is usually high, or very high.

    Hang in there best you can and trust your Doc. Sounds like your Doc is keeping you on an every 2 week appt schedule so he/she can keep close tabs on you, which is good.

    Kass, by muddling through I mean just trying to be patient, not try to let it get me down, which can be hard, as I am sure you know. One day at a time, sometimes it gets hard to do that, so yes I try to muddle through the pain & the days, one day at a time. If I try to look to far ahead I get overwhelmed with the what ifs, you know what if this don't work & so on.... I have been & am in your shoes, hardest thing not knowing fully what is wrong, at least if you knew everything you could try to deal with it, but sounds like your doc. is on the right road, the most important thing at this time while you are in the process of finding what exactly is going on is pain control, you deserve quality of everyday life & good pain control will give that, perhaps not like before but better then not having any control at all.
    I have low BP to begin with so it does not take much to drop my blood pressure. We can all react different to meds.I truely wish I had done better on the oxy.
    As for the osteo. treatment I am waiting for the office to call, they said it can take a few days just to call you back for an appointment. My surgeon told me he believes there is a waiting list to get in. Great, I am going to ask him if he can get the ball rolling to try to figure out if the ribs are messed up so that I don't wait months just to get a diaganosis. They are extremely painful & so is the chest, which can make it difficult to breathe & move certain ways, even sleep. I just hope the I have luck if that is the case with the manipulation of the ribs. I have been in pain for almost three years & it is really getting to me, since surgery it is worse then ever. So I am glad to have some positve feedback from you on the osteo. treatment. I feel for you & hope something is found soon, perhaps something will show in the bloodwork. Hang in there, we all try to help each other through, I honestly believe as much as our families love us they just cannot identify with being in severe pian everyday, but the people here can & that my friend is a true blessing. Please let us know how it is going & yes I may very well have some questions for you, thank you for being kind enough to take the time to answer them. Sammy

    What kind of doctor does this? It almost sounds like a chiropractor who does manipulations of different areas. Just curious. I used to see a chiropractor after injuring my back a couple of years ago and he did wonders for me especially this trigger point massage therapy. I know what you mean about the pain sometimes being worse the next day before it gets better. Now, I do the trigger point massage when I have really sore muscles and I feel the "knot" in the muscle. It hurts like heck but it works.

    Kass- I too am from Maine and would love to find out which Dr. you are seeing, I have been diagnosed with FMS but am wondering if it is actually MPS. I'd really like to find a doctor with experience in dealing with this. Some days I am in so much pain I can't function which is driving me and my family crazy.

    bccled: Since this is an old post, I would suggest you post a new thread of your own. You'll get more responses to a new one than by adding questions to an old one. I will answer as best as I can in this one.

    First, welcome to the boards. I have chronic myofascial pain and it shares many symptoms with fibromyalgia. Some people actually are diagnosed with both. They are 2 separate conditions, but unfortunately they can be diagnosed incorrectly because many doctors either don't know much about them or use them to diagnose a patient when they can't find anything else to diagnose.

    The type of doctor I see for my CMP is a Physical Medicine and Rehabilitation Specialist (PM&R) also known as a Physiatrist. He is an MD and treats chronic conditions and acute conditions as well. He is very well versed in this area and has managed my pain for 8 years now. They are not always available in smaller cities and towns. Mine is about an hour away.

    I'd be happy to answer any questions you might have. Just start a new thread, and ask your questions. There are others here who have fibro and myofascial pain and I'm sure you can get lots of info.

    bccled, welcome. I am glad I checked out this thread. I am glad you posted. Where do you live in Maine? You can give me an area if you do not want to say your city. I live in Bangor, and I have a great doctor. As I have written in my other posts, I have mps, and fibro too. It kind of all happened in the last 8 months. I have learned a lot about the difference between chronic myofascial pain and fibromyalgia. It really stinks to have them both and battle the chronic pain of those and tmj, and migraines.

    Please write me back, and I can tell you as much I can. Or you can start a new thread either here or in the fibro board.

    Thank you all for responding. I need as much help and education as I can get. I am so greatful for your responses. I have now been dx with fibro which explains the overall body pain and the extreme stiffness in the body. I had an idea I had it, I have all the trigger points and I feel like 1 big bruise. I also feel the rope like masses in my neck and shoulders from the Mps. I am just a mess. But I am now on 150 mg 2x daily of Lyrica, and I hope that helps with the overall pain. I also am trying to take the muscle relaxants 3x daily as I am prescibed.

    I know this might sound extreme, but my doctor and mostly I have decided to wean off the oxycontin. I barely got the pain under control and now I am going off. I am anxious and scary to see where my pain level is. I am now on 40 mg during the day. I stopped taking my nightly dose, and notice the jaw,face, teeth pain is coming back. I was never able to get the surgery due to my insurance made a mistake telling me I had the coverage, and it turned out I did not. I do not know how to battle it, and I have asked for so much help, even traveled to other states, and every one classifies it as tmj, and insurance, dental and health, does not cover tmj. I want it to be named something else, and I even ask them to help me as my insurance will not cover it, and they alway say tmj. I am hoping the tmj has flared down for now.

    I want to find other ways to battle my pain, and use the medications only when I absolutely need it. I have high hopes, but I also am realistic that I am not sure that is possible. I will keep you all posted on how my weaning down goes and where my pain is. When will I know my pain is actually true pain, and how do I manage if my brain is telling me it is worse than it actually is?

    This morning I took my 40 mg o.c. at 8am, and 9:30 I was in a lot of pain as I usually am in the morning. I find I always have to take bt in the morning. My hardest times are mornings and around dinner time. I am going to try and rest today.

    Hi Kass: I'm a little torn about you going off the meds. I know you've had some issues because of them, but please don't suffer in silence, especially when it took so long to get relief. I can understand that you at least want to try, but leave yourself an open door, in case you need to go back.

    Was it you or the doc who brought up weaning off? Has he let you know that you have the option to go back on the meds, if you can't take the pain? I'm just worried about you, hon. You say you have now been diagnosed with fibro. Is it your GP or a PM who diagnosed it? I'm just asking, because if that can be listed as your primary diagnosis, instead of the TMJ, maybe it will help with the insurance issues.

    I will always agree that other modalities, like PT and massage are a good idea, but I feel when you are chronic, it's not beneficial to take occasional meds. That may be OK for pain that is a once in a while type of thing, but to me, chronic pain requires chronic meds. Just my opinion. As always it is your body and your choice. I would never want you to think I am pushing you to do something you don't want to. I just don't want you to close that door, in case you need to go back.
